11. How to fill out the survey Go to the Wellness & Prevention Office website: www.wellnessandpreventionoffice.org. Click on the white CSHT Assessment Tool tab at the top of the page on the right side. When you click on the CSHT Assessment Tool it will take you to the instruction page, print a hard copy of the assessment tool to use as a worksheet with your CSHTeam. (See red notation under item # 2 to print hard copy). Complete and submit only one on-line assessment tool. To save a hard copy of your on-line responses for your records, print each page before you proceed to the next page. You must identify the CSHT Chairperson and all committee members on the first page. You must hit Submit at the bottom of the final page before logging off.
