3 2011-12 Employer Contribution Both the state and the A&M System contribute a portion of the total cost for Health and Basic Life coverage If you waive health coverage and certify that you have other health coverage, you will have $183.36 (full-time) and $91.68 (part-time) to use toward optional coverage Part-time employees (working 20 to 39 hours a week) receive one-half the amount provided to full-time employees

5 Premium Changes o Premiums will decrease for: o Vision o Long Term Disability o Dental DHMO o Premiums will remain the same for: o A&M Dental (PPO) o Premiums will increase for the A&M Care and Scott & White health plan. o Premiums will remain the same for all other optional coverages

6 September 1, 2011 Health Plan Changes HMOs o First Care Health Plan o First Care will no longer be available as of 9-1-11 o Enrolled members who do not make a change during annual enrollment will be put into the A&M Care plan as of 9-1-11.

7 September 1, 2011 Health Plan Changes Scott & White Health Plan o Services, other than office visits, will have a $350 deductible and will be paid at 80%. o Primary Care office visits increase to $30. o Specialty Office Visits will be $45.

8 September 1, 2011 Plan Changes A&M Care Plan o Only one A&M Care plan will be offered for Active Employees o $700 Deductible o 70%/30% Coinsurance o Out of Pocket Maximum: $5,000 individual and $10,000 family

9 September 1, 2011 Plan Changes A&M Care Plans – Continued Annual Preventive Exams will be covered at 100% Deductible credit will continue to be provided for completion of a Health Risk Assessment (HRA) and completion of an annual physical. If you, your spouse or a covered dependent, age 18 or older, completes an HRA at www.bcbstx.com, $50 will be credit towards that member’s deductible. As each family member visits a physician for an annual physical, another $50 will be credited toward the deductible.www.bcbstx.com

10 September 1, 2011 Plan Changes Dental o Small reduction in premium for Delta Care USA (DHMO Plan) and, o Slight increase in copayments for some services o No plan or to premium changes to the Dental PPO Plan

11 September 1, 2011 Plan Changes Vision Plan New Carrier – EyeMed Vision Care Reduced Premiums Enhanced Benefits Materials copay $15 40% discount off additional materials Increased out-of-network benefit Explanation of Benefits for every claim filed

13 To change your benefit elections or enroll or re-enroll in Spending Accounts: o Employees make enrollment changes online o Retirees may make changes online or use the Personal Benefits Summary letter Deadline for changes: July 31 Print a statement of your benefits.
