Topics: What is imMTrax Memo sent to birthing hospitals May 16, 2013 Hospital worksheet Electronic Birth Certificate System Screenshot Reminder- *Consent to immunize is different than consent to share information!!

4 #7-Vital Statistics Pre-Register and/or Baby is Born at Hospital Nurse and/or Parent Fills out the paper Worksheet Personnel at hospital enters into Electronic Birth Certificate System Once entered into electronic system it uploads monthly into imMTrax Baby is seen for 2 month check up and imMTrax should have birth dose and consent Process:

5 #7-Vital Statistics Pre-Register and/or Baby is Born at Hospital: Use the 2013 updated worksheet Fill out information completely 2 pages (front and back) to complete

6 #7-Vital Statistics Hospital Worksheet: Consent Obtained (front page) These fields should be completed by the parent This field MUST be filled in for the birth dose to transfer to imMTrax. Hep B Vaccination Information – Infant/Mother These fields should be completed by hospital staff If you do not know if they gave consent, ALWAYS mark “Unknown” or leave blank, DO NOT mark “No”. If no is marked it hurts the whole state of MT to review records. This response should match, if not marked mark “Unknown”

7 #7-Vital Statistics Personnel Entering into Electronic Birth Certificate System: All doses of Hepatitis B should be entered into the electronic system

8 #7-Vital Statistics Birth dose comes into imMTrax: Once entered into the electronic birth certificate system it comes into imMTrax the IIS for the State of Montana

9 #7-Vital Statistics Hospital 1 - Doing a Total GREAT Job! Hospital 2 - Doing a GREAT Job! Hospital 3 – NOT Doing a so great! Hospital 4 – Not entering correctly A look at the data: Table Shows Total Births Total HepB Given Consent Yes, No, Blank Better to leave blank or Unknown then to mark NO. We compare # of doses shipped to the hospital to number of doses administered to babies

10 #7-Vital Statistics Summary: Please use current worksheet (2 sides) Fill out completely with all information Make sure grayed areas are marked with the correct information If consent is not marked, leave blank or mark unknown Enter into the electronic birth certificate system Data gets uploaded to imMTrax The process is complete and we can assist all clinics with getting the birth dose enter into imMTrax for the child's 2 month check up!
