2 Coverage for MHS Members LCP, a long time partner with MHS, provides quality transportation services for all eligible MHS members per IHCP coverage guidelines. The following “Enhanced” services are also offer a value added services;  No trip limits  Additional rider availability  Trips to and from Caseworker appointments  Trips to the Pharmacy (during normal transport route)  Special MHS health related events  Bus or mileage reimbursement options www.lcptransportation.com

3 Trip Scheduling To Schedule Transportation call: 1-877-MHS-4U4U(647-4848); For Return Trips call: 1-877-MHS-4U4U; After Hours/ Urgent Care Trips 1-800-508-7230 or 1-888-732-2097; 1-800-508-7230 or 1-888-732-2097; All Non-Emergency Transportation Services Must be Scheduled between 8:00a.m. and 5:00p.m. Monday thru Friday; Trips May be Scheduled out 30 Days in Advance: www.lcptransportation.com

4 Specialty Trips All Trips Non-Emergency Ambulatory and Non- Emergency Non-Ambulatory (Wheelchair) are scheduled by 4:00p.m. Two (2) Business Days in Advance; Also Trips over 50 miles are scheduled by 4:00p.m. Two (2) Business Days in Advance: www.lcptransportation.com

5 Urgent Care Trips Urgent Care Trips can be provided Same Day if the PMP confirms that the Trip is Urgent in nature; Urgent Care Trips have a Two Hour Window for Pick-up from the PMP’s Confirmation Time; Return pick up time is 1-1:15 hours depending on location. www.lcptransportation.com

6 Safety/Accomodations Minor Children ages 0 thru 17 must be accompanied by a 21 year old Parent or Guardian Escort or Authorized Representative; Car Seats are Required by Law for Children Newborn to Eight Years of Age; LCP Provides Booster Seats for Children 4 Years of Age and 40 Pounds to Eight Years of Age; Prior Approval is Required for Out-of-State Trips www.lcptransportation.com

7 Business Requirements LCP only Transports MHS Members to Medicaid Covered or MHS Authorized Services; Trips to the pharmacy are allowed when leaving the provider’s office for prescription pick-ups; MHS Members being referred must have the Facilities Name, Address, and Phone Number in order to Schedule the Trip; www.lcptransportation.com
