3 Blood Glucose Levels Daily monitoring of your blood glucose is critical to understanding how well you’re managing your diabetes on a daily, and even hourly, basis. Keep a record of your meter readings and review the numbers with your diabetes care team during each healthcare visit. Recommended blood glucose numbers Before meals: 1-2 hours after meals: 70 – 130 mg/dL less than 180 mg/dL A1C Test Your A1C is a simple lab test that reflects your average blood sugar level over the previous three months. It’s the best way for you and your diabetes care team to know how well your treatment plan is working. An A1C of less than 7% will decrease your chances for serious health problems. Level of Control Normal Your Goal Need to Take Action A1C Number 6 % or less less than 7 % 7 % or more My A1C:My Goal A1C: Blood Pressure By measuring your blood pressure, you can tell if your heart is working harder than it should to circulate your blood, which can cause health problems. If you have diabetes, you should aim for a blood pressure reading of less than 130/80 mmHg. My Blood Pressure: / My Goal Blood Pressure: / Know your numbers to prevent complications The target numbers listed on this handout are general recommendations for people with diabetes. However, you and your healthcare provider will work together to set personalized target goals in each of these areas. DI83923PAR Sponsored by Copyright © 2011. 4 Cholesterol Cholesterol has three separate parts that can be measured in your blood: Low density lipoproteins or LDL, High-density lipoproteins or HDL, and triglycerides. If levels of LDL are too high, this can lead to build-up of cholesterol in your arteries. HDL, or “good” cholesterol, helps remove cholesterol from your blood. Triglycerides, blood fats, may increase your risks as well. It’s important that all parts of your cholesterol are within healthy levels. For most people with diabetes, cholesterol goals are: LDL - less than 100 mg/dL HDL - greater than 50 mg/dL for women and 40 mg/dL for men. Triglycerides - less than 150 mg/dL My Current Levels (mg/dL)My Goal Levels (mg/dL) LDL HDL Triglycerides Know your numbers to prevent complications Continued...
