3. Trips may be scheduled out 30 days in advance: –All trips, non-emergency ambulatory and non- emergency non-ambulatory (wheelchair), must be scheduled by 4:00 p.m. two (2) business days in advance –Trips over 50 miles: must be scheduled by 4:00 p.m. two (2) business days in advance, and require prior authorization.

4. Urgent Care trips can be scheduled same day if the PMP confirms that the trip is urgent in nature Urgent care trips have a two- hour window for pick-up from the PMP’s confirmation time

5. Minor children ages birth thru 17 must have parental or guardian escort or show proof of legal emancipation Car seats are required by law for children newborn to eight years of age LCP provides booster seats for children 4 years of age and 40 pounds to 8 years of age Prior approval is required for elective procedures such as: –Bariatric –Out-of-State Trips

6. LCP only transports Anthem members to Medicaid covered or Anthem authorized services Anthem members being referred must have the facilities name, address, and phone number in order to schedule the trip
