4 Short Term Medical STM – What is it? –Affordable major medical coverage purchased on a temporary basis –Accept/reject –Effective dates as early as 12.01a.m. the next day. Can request later effective date up to 60 days after application date

5 STM eligibility Eligible applicants ages 18 through 64 years and 11 months, spouses, and dependent children under age 19 (under 25 if full time student) Child only ages 2-18 Applicants can answer health questions with ‘NO’ response

6 Payment Options Monthly pay – up to 6 or 12 months –Check, money order, credit card, auto bank withdrawal Single pay – 30 to 180 days, select exact number of days 10 day free look period

7 Payment options Your subsequent monthly credit card/ach premium deductions will occur based upon the effective date of coverage. –If your coverage effective date is the 1st -15th, your premium will be deducted on the 1st of the month. –If your coverage effective date is the 16th- 31st, your premium will be deducted on the 15th of the month.

8 STM Plan Enhancements Ambulance; now up to $500 per occurrence for ground and up to $1,000 per occurrence for air. Removal of $2,500 benefit maximum for knee injuries/disorders and gallbladder removal surgery; benefit now pays up to the calendar- year maximum Removal of the six-month waiting period for hysterectomy, tonsillectomy, adenoidectomy, repair of deviated nasal septum or any type of surgery involving the sinus, myringotomy, tympanotomy, herniorraphy or cholecystectomy

11 Instead of a calendar-year deductible, this plan offers a choice of daily maximums: $250, $500, $750 or $1000. Plan participants pay for the first dollar of covered expenses up to their chosen daily maximum; Plan pays 100% of the expense in excess of the daily maximum for all covered expenses $4,000 calendar-year out-of-pocket maximum; every dollar spent on daily maximum expenses applies to the $4,000 out-of-pocket maximum

Within the past 5 years have you or any person applying for coverage been aware of, received an abnormal test report for, been diagnosed with, been treated by or received follow-up care with a member of the medical profession, taken medication for or had a device surgically implanted or in place for: ■ heart disorder, heart attack, coronary artery disease, coronary bypass or stent ■ peripheral vascular disease or carotid artery disease ■ stroke or other neurological disorder, cancer or tumor ■ paraplegia, quadriplegia or multiple sclerosis, stem cell transplant ■ emphysema or COPD (chronic obstructive pulmonary disease) ■ diabetes, liver disorder ■ kidney disorder other than stones ■ degenerative disc disease or herniated disc ■ rheumatoid or psoriatic arthritis ■ degenerative joint disease of the knees or hips ■ alcohol or drug abuse or dependency ■ hemophilia 7. Have you or any person proposed for coverage been diagnosed or treated for Acquired Immune Deficiency Syndrome (AIDS), AIDS-related complex, or any other immune system disorder? Answer this question “no” if you have tested positive for HIV but have not developed symptoms of the disease AIDS
