2  Chapter 30 Montgomery GI Bill-Active Duty: 2 years active duty, 3 years active duty, or 2 years active duty plus 4 years reserves.  Chapter 31 Vocational Rehabilitation and Employment.  Chapter 33 Post 9/11 GI Bill: Servicepersons who served on active duty for at least 90 aggregate days after 09/10/01  Chapter 35 Dependents’ Educational Assistance: Educational Assistance paid to dependents of Veterans who are permanent and total disable or died as a result of service connection (Child, Spouse and Surviving Spouse)  Chapter 1606 Montgomery GI Bill- Selective Reserve: Completion of initial active duty for training. Must be active reservist with a 6 year obligation  Chapter 1607- Reserve Educational Assistant Program: 90 days or more active duty support of contingency operation or full-time National Guard duty responding to nation emergency.

3  DU’s Bursar Office does the billing  At the 100% charge period for DU the Bursar Office bills VA (approx. 3-weeks into the term)  Payment is received from VA and applied accordingly  Refunds are issued based on the credit balances

4  All VA paperwork comes to the Registrar’s Office email account  Paperwork comes from advisors and/or veterans  The VA paperwork goes into a Queue to be processed by any certifying official  Certifying Official fills out certifying form and puts paperwork directly into ImageNow (DU’s imaging system) student VA file

5  We developed our own form to help in processing VA paperwork  Form has all pertinent information to allow for ease of processing in Banner and in VAOnce  Coding in Banner allows for pertinent information about the veteran’s classes to appear on a WebFocus (DU’s reporting system) to allow for the certification of credits in VAOnce.

8  Use report to certify credits in VAOnce  Report shows delivery options by color to aid in certifying  (Part of Term- PoT)  1 – 15 Week Classes  SE1 – 1 st Seven Week classes  SE2 – 2 nd Seven Week Classes  S10 – 10 Week Classes  Report shows start and end dates of each delivery option

9  Every 3 to 4 weeks run a new report and review changes in the following:  Credit hour changes  Delivery option changes  Starting day of class changes  Drops/Withdrawals  Non punitive grades  Failed Classes  Punitive grades  Make changes in VAOnce

10  Run final VA report in WebFocus  Report any final grades  Earned F’s need not be reported  Incomplete Grades (I’s)  Report Probation/Suspension through Right Now Web  After Final Graduation Audits are Complete  Report Graduation through VA Once  Terminate last certification

11 Draft of form that will be used this coming Fall semester. I UNDERSTAND THAT I HAVE THE FOLLOWING OBLIGATIONS WHILE ATTENDING DAVENPORT UNIVERSITY AND RECEIVING VA EDUCATIONAL BENEFITS: Please read and initial each bullet point below: I understand that I must have relevant VA paperwork on file with Davenport University. Relevant paperwork includes: VonApp confirmation page until a Certificate of Eligibility is available, a 22-1995 form if I attended any other college in which I received VA benefits and/or a 28-1905 for Chapter 31 students. I understand I must have Official Transcripts sent to Davenport University from all of my previous colleges. I must be pursuing a program of study and making satisfactory progress toward completion of the degree listed above. I understand that the VA will only pay for classes that apply toward an approved educational major. If I decide to change my desired major, I must notify, via email, the Central Registrar’s Office in writing (see below) and attach a Curriculum Update form. I understand that I must notify, via email, the Central Registrar’s Office (see below) of any change in enrollment. These changes include any increase or decrease in credit hours, delivery option change and/or in seat vs online class changes. I understand that if I withdraw from or stop attending a class (receive a W, F or NF), Davenport University must report this grade along with the last date of attendance to the VA. Further, I understand that in either case, I could be responsible for the repayment of VA benefits associated with these grades. Repayment could include book stipend, monthly housing allowance and tuition and fees. I must also notify, via email, the Registrar’s Office of any such withdrawal (see below).

12 I understand that if I am required to take a developmental class, the class must be taken in seat at a Davenport University campus. *Note—Chapter 31 recipients are approved to take developmental classes online as well as in seat. I understand that if I attend until the end of a class and take the final exam and receive a failing grade, the F does not need to be reported to the VA. The VA will allow a repeat of any class in my major that was not successfully completed (F, W or I grades). However, the grade will effect my GPA and could impact satisfactory progress. Satisfactory progress must be maintained as stated in Davenport University’s catalog. I understand that I cannot repeat a course that I have already successfully completed. I understand that if I am called to active duty I am entitled to a “Military Withdrawal”. In this event, I must deliver a copy of my orders to my academic advisor. I understand that the VA requirements for full time status may differ from the Financial Aid and/or the Davenport University’s requirements for full time status. It is my responsibility to know the difference. I understand that if I am a Chapter 33 veteran, my credit hours will be certified at the beginning of the semester, however my tuition and fees will be reported at the time of the 100% charge in accordance with University Policy. I, the undersigned, do hereby affirm that I have received, understand and will comply with the “Veterans Statement of Responsibility”. I further affirm that I understand I will be liable for any debt letter and/or overpayment to me caused by my failure to adhere to the “Veterans Statement of Responsibility” and will advise Davenport University Certifying Official of any changes in my enrollment.

Copy of ledger of each student account (i.e. billing statement) showing tuition and fee charges, and payments for all semesters. Printed copy of curricular requirement for each student's academic program(s) of study. Registration document(s) showing class schedule, type of class (residence or distance), and charges for each term (per class). VA enrollment certifications for review during site visit (historical chronological order of student’s VAOnce, 1999’s and 1999b’s). Provide annual amount of tuition and fees of Yellow Ribbon recipients (if applicable). Copy of school catalog(s) available for the periodicity of Fall 2010 to present. Copy of enrollment or billing statement verifying tuition and fees paid by a similarly situated non-VA student. One non-VA student academic file, any degree, for review during site visit. School/facility “Advertising file”. *If applicable: VA Work-study student files; to include all VA Work-study students employed from Fall semester 2010 to present. 14  DU’s compliance review was just last week  The auditor reviewed 25 files  We passed our review  Cannot have more than 50% of errors and no more than 30% concerning money  If more errors than are allowed, auditor expands the review and looks at more files  Auditor writes up findings and sends a report
