11 G ENERAL I DEA We are focused on each time a material or Item is used. Referred to as a History Each Item is stored with its properties and attributes, (Safety precautions, Waste Class etc.) Materials never used before will be added to a growing list of Items A single Item can be paired with many Histories, (not the other way around) Items occurring more than 150 times/ year are designated as Frequently Used Items (FUI)

12 I NVENTORY M ENU D ETAIL

13 P ROCESSING A H AZARDOUS M ATERIAL Making an entry on this form will add a new item to the database. Once picked up, the new item is logged into receiving and converted to inventory “New” does not mean it has never been used before. It refers to a new collection.

Use the previous Green Form to simply process a new item occurrence Yes, there are 2 separate Green Forms: One for adding New Entries or non FUI materials One for adding New FUI usage.
