1 2014-2015 At Risk After School Supper Program Site Monitoring Training for Food Services Staff Provided by the LAUSD Food Services Division 08.01.2014

2 Overview This training will provide attendees with the knowledge to understand the Supper program processes and requirements. This training will address the following areas: What is After School Supper Required documentation Role of the Food Services staff Monitoring requirements Responsibilities of the Afterschool Program staff

3 Overview Why are site monitoring conducted? What are the monitoring requirements? What are the corrective actions requirements? What is the required documentation?

4 Our goal is to conduct the three (3) required site monitoring on a timely manner to ensure the program is in compliance with the USDA and the CDE rules and regulations Provide necessary corrective actions in accordance with the site monitoring findings Conduct follow-up training as needed Goal

5 Annually Monitoring Requirements The Food Services Manager or designee is required to conduct: 3 mandatory site monitoring to ensure the program is following proper procedures 1 site monitoring may an announced 2 site monitoring’s must be unannounced If corrective actions are needed additional follow up monitoring's will be conducted Coordinate with the AFSS and communicate any concerns or issues discovered during the monitoring

6 Procedures for Corrective Actions Corrective actions Any corrective actions require Follow up must be completed within 60 calendar days Discuss the monitoring’s finding and any necessary corrective actions with the afterschool program coordinator. Provide a copy of the monitoring to the after school program coordinator. All site monitoring’s and corrective action documentation must be filed and keep for 3 years.

7 Monitoring Timeline The first monitoring 4 weeks after the start of program Completed before September 10, 2014 No more than six months may lapse between reviews The second monitoring must be conducted prior to winter break Completed before December 19, 2014 The third monitoring must be conducted prior to spring break Completed before March 27, 2015

8 Submission of Monitoring Reports Send a copy of the monitoring to the Beaudry Building - Food Services, 28 th Floor. Attention: Food Service Staff Aide (per each ESC) North ESC – Lisa Hess South ESC – Olukemi (Felicia) Coker East ESC – Abyz Dayrit West ESC – Edna Gabriel Give a copy to the program coordinator File and keep for 3 years

19 Summary of Findings and Recommended Corrective Actions 5 Meals were observed to be taken offsite by students- Recommended corrective action reinforce with afterschool program coordinators and staff that meals must be consumed on site. Meals were not recorded on the reimbursable meals worksheet at the time of meal service.
