All rights reserved. 3Qwest Confidential & Proprietary Release 14.0 – QORA ASR Gateway Qwest will implement the ASOG Version 39 Industry Standard changes to the ASR forms and fields decided upon at an industry level with participating members of the OBF Committees. Effective date for this release as dictated by the industry is September 21, 2009. For detailed information about the industry changes on the ASR – refer to the ATIS website: http://www.atis.org/http://www.atis.org/ Qwest will implement upfront validations in the QORA ASR Gateway that impact all ASRs being transmitted to Qwest electronically. This includes the web-based QORA (Qwest Online Request Application) GUI application, or through direct connections, such as NDM or UOM interfaces.

All rights reserved. 4Qwest Confidential & Proprietary Customer Impacts/Value How does implementation of Qwest up front editing add value for you, our customer? –The QORA GUI application is a superior alternative to paper ASR forms, offering on-line help as well as validations for accuracy. –Upfront edits streamline processing time for your request, by ensuring the data supplied by you is as accurate as possible. –Prior to the ASR being accepted into Qwest’s systems, you are notified of the errors on the ASR allowing corrections to be made, thereby reducing the need for costly re-issues of the ASR (supplements). –An accurate ASR helps ensure the accurate provisioning of your request the first time. –Accurate entries also create accurate billing, reducing the possibility for errors on your monthly bill. –Reduced cycle time due to less re-work and supplements being required.

All rights reserved. 5Qwest Confidential & Proprietary General Changes Pre-Order Validation Enhancement – QORA Gateway –WKTEL format Validation – The Pre-order address validation by WKTEL service will no longer accept the non-industry standard WKTEL format of 12-characters (embedded dashes) and instead ONLY accept the industry standard format of 10- characters (no embedded dashes). QORA GUI and ASR Gateway Validations: –Qwest is implementing an edit to validate the PNUM field on the ASR form for all Forbearance impacted products for special pricing. Products include (Asynchronous Transfer Mode (ATM), Frame Relay (only speeds of 200 Kbps or higher), GeoMax®, HDTV-Net service, Qwest Metro Optical Ethernet (QMOE®), Synchronous Service Transport (SST), Self Healing Network Services (SHNS), Ethernet Ports over SONET (EPoS) associated with SST and SHNS. The PNUM field must be populated with the ILINK Number from the pricing exhibit contract when special pricing is expected to be applied on the service being requested. If the PNUM is invalid the ASR shall be rejected. –SUP = 2, Qwest shall validate on supps for a due date change, that the only fields changed are the ASR.DDD and ASR.EXP if applicable. If additional fields have been changed, the ASR will be rejected.

All rights reserved. 6Qwest Confidential & Proprietary CCEA/SCCEA Validation: –Qwest will perform an availability check on the CCEA/SCCEA field on all product specific forms, e.g., Trunking, Transport, and the Additional Circuit Information form for original submission of the ASR and on SUP types 2, 3, 4. If the terminations requested are busy or not found, the ASR will be rejected. CFA/CCEA Validation: –Qwest will reject the ASR if both the CFA and CCEA fields are populated on all product specific forms and ACI forms. CFA/SCFA Validation: –Qwest is implementing an edit to validate on REQTYP = “M” that the SCFA is a lower speed than the CFA. If the SCFA is higher or equal to the CFA, the ASR will be rejected. Optical CFA Validation: –Qwest will validate the CFA, when the CFA is populated with an Optical facility name (second delimited position starts with OC) on all product specific forms and Additional Circuit Information forms. If the circuit is BUSY or the CFA is not found, the ASR will be rejected. Field Rules/Validation

All rights reserved. 7Qwest Confidential & Proprietary TCIC Validation: –Qwest will validate the total number of TCICs on the Trunking and ACI forms equal the total number of the Trunks shown in the QTY field on the ASR form. If the fields do not match, the ASR will be rejected. SFNI Validation : –Qwest is adding new criteria of ACT type = N or C to the existing rules for the SFNI validation to bring in line with ASOG usage rules. SMUXLOC Validation : –The SMUXLOC field will be required on the Transport, Trunking and End User Special Access forms when the CFA data provided indicates an optical facility as determined by the following criteria:  Pos 1 and 2 of the CFA entry equal HN or GM  Pos 1 of the second delimited position of the CFA entry equals “W” or “OC”  UNE is blank  ACT field on the ASR form is N, C or T Field Rules/Validation

All rights reserved. 8Qwest Confidential & Proprietary “Report” Tab : –Qwest is adding a new checkbox item called “INIT” in the “Information to Display” section on the Reports Tab in the QORA GUI that when checked will display the INIT (initiator of the ASR) on the report. All rights reserved. 9Qwest Confidential & Proprietary Customer Responsibilities Arrange to participate in the 30-day testing window prior to the release –Test for the ASOG 39 Industry changes per industry test plan. –Test for the new Qwest ASR Gateway upfront validations and rule edits Ensure information on the release details is shared internally with those employees issuing ASRs to Qwest.

All rights reserved. 10Qwest Confidential & Proprietary FAQs Are manually transmitted ASRs impacted with this release? –Yes – the industry changes for ASOG Version 39 are applicable and the new forms must be used effective with ASRs transmitted to Qwest after 3:00 pm (MST) on Friday September 18, 2009. Do the QORA ASR Gateway upfront edits impact our company if we have our own ordering systems? –Yes – all ASRS electronically submitted to Qwest through direct connections, e.g., NDM or UOM are impacted with the new edits/validations.
