2 Job expectations Serves as a resource for clinical staff Visits all sites of service (Nursing Home and Home patients) for review of all QA measures as relevant to development of instruction Educates staff in clinical aspects, as needed Communicates appropriate clinical information to the nursing home staff and interdisciplinary team regarding hospice care issues and patient/family needs Thinks and acts calmly and logically to meet unusual occurrences of the job Interfaces with patients, families, and staff to ensure customer satisfaction Communicates effectively with patients, families, and other health care providers Maintains clinical competency by assuming responsibility for acquiring knowledge and experience to meet learning needs

3 Job Functions Develops and promotes programs for continuing education for all interdisciplinary team members at local offices Provides assistance with development and evaluation of hospice programs through annual agency evaluations Provides in-services for nursing home staff, and community resources regarding hospice services and Medicare guidelines regarding hospice appropriateness Communicates appropriate clinical information to the nursing home staff and interdisciplinary team regarding hospice care issues and patient/family needs

4 Teamwork and the New Hire Our organization – Patients Reside in homes, nursing facilities, assisted care facilities – Doctors, nurses, social workers, bereavement counselors, chaplains and volunteers Provide care for patients with end of life issues Must work together to ensure best care for patients

5 Gathering Data We need to provide a job description – Interview current staff regarding their jobs Include all parts of the job; both good and bad – Identify what we would ideally like in applicant if chosen Clarify need to work with patients in sensitive manner Desire to aid families in difficult issues Consideration for maintaining facility’s excellent reputation
