2. *Emperor of Heaven's Special Pill to Tonify the Heart (tian wang bu xin dan) C:Sheng Di Huang D:Dan Shen, Dang Gui, Bai Zi Ren, Yuan Zhi, Ren Shen, Fu Ling A:Tian Men Dong, Mai Men Dong, Xuan Shen, Wu Wei Zi, Suan Zao Ren E:Jie Geng, Zhu Sha Pattern: Yin deficiency of the Heart and Kidneys. Indications: Irritability, palpitations with anxiety, fatigue, insomnia with very restless sleep, inability to think or concentrate for even short periods of time, nocturnal emissions, forgetfulness, dry stools, a red tongue with little coating, and a thin, rapid pulse. There may also be sores of the mouth and sores of the tongue, low-grade fever, and night sweats.

3. This formula and Restore the Spleen Decoction (gui pi tang) are both used in treating conditions marked by insomnia, palpitations, and forgetfulness. However, the latter formula is indicated for concurrent qi and blood deficiency marked by reduced appetite, loose stools, a pale tongue, and a thin, frail pulse. This formula is for blood and yin deficiency with irritability, a red tongue with little coating, and a thin, rapid pulse.

4. *Sour Jujube Decoction (suan zao ren tang) C:Suan Zao Ren D:Chuan Xiong A:Fu Ling, Zhi Mu E:Gan Cao Pattern: Liver blood deficiency with heat from deficiency. Indications: Irritability, inability to sleep, palpitations, night sweats, dizziness and vertigo, dry throat and dry mouth, a dry, red tongue, and a wiry or thin, rapid pulse.

5. Settle the Emotions Pill (ding zhi wan) C: Ren Shen, Fu Ling Shi Chang Pu, Yuan Zhi Pattern: Deficient Heart Qi with severe emotional shock Indications: Apprehensiveness, easily frightened or incessant laughter, palpitations, anxiety, forgetfulness

6. *Licorice, Wheat, and Jujube Decoction (gan mai da zao tang) C:Fu Xiao Mai D:Gan Cao, Da Zao Pattern: Restless organ disorder (zang zao); injury to the Heart yin, Liver qi, and Spleen qi. Indications: Disorientation, frequent attacks of melancholy and crying spells, inability to control oneself, restless sleep (sometimes with night sweats), frequent bouts of yawning, a red tongue with a sparse coating, and a thin, rapid pulse. In severe cases, the behavior and speech become abnormal. This is called restless organ disorder (zang zao), an emotional condition that is generally due to excessive worry, excessive anxiety, or excessive pensiveness.

Bupleurum plus Dragon Bone and Oyster Shell Decoction (chai hu jia long gu mu li tang) C: Chai Hu, Huang Qin, Ban Xia, Ren Shen, Sheng Jiang, Gui Zhi, Fu Ling, Long Gu, Mu Li, Da Huang, Da Zao, Qian Dan Pattern: Disorder of all three Yang stages or extreme fright Indications: Fullness in the chest, irritability with occasional palpitations, urinary difficulty, constipation, delirious speech, inability to rotate the trunk, sensation of extreme heaviness throughout the body, red tongue with slippery coating, wiry rapid pulse

9. *Two-Cured Decoction (er chen tang) C:Ban Xia, Ju Hong (or Chen Pi) D: Fu Ling A:Zhi Gan Cao E:Sheng Jiang, Wu Mei Pattern: Damp-Phlegm due to Spleen's failure to transform fluids Indications: Coughing with copious, white sputum that is easily expectorated, focal distention and stifling sensation in the chest and diaphragm, palpitations, nausea or vomiting, dizziness, a swollen tongue with a white, thick, greasy coating, and a slippery pulse.

10. CAUTIONS AND CONTRAINDICATIONS: The improper use of this formula can lead to excessive thirst and a dry throat. Unless modified, it is contraindicated for cases with cough associated with Lung yin deficiency.

11. *Warm the Gallbladder Decoction (wen dan tang) C:Zhu Ru D:Zhi Shi A:Ban Xia, Chen Pi, Fu Ling, Gan Cao E:Sheng Jiang Pattern: Gallbladder-Stomach disharmony with Phlegm- Heat Indications: Dizziness, vertigo, nausea or vomiting, insomnia, palpitations, anxiety, indeterminate gnawing hunger, or seizures accompanied by copious sputum, focal distention of the chest, a bitter taste in the mouth, slight thirst, a greasy, yellow tongue coating, and a rapid pulse that is either slippery or wiry.

Minor Sinking into the Chest Decoction (xiao xian xiong tang) Gua Lou, Huang Lian, Ban Xia Pattern: Clumping in the chest due to Phlegm-Heat Indications: Focal distention (with or without nodules) in chest and epigastrium tht are painful when pressed, coughing of yellow, viscous sputum, constipation, bitter taste in mouth, greasy yellow coating, Slippery, rapid or floating Pulse.

14. *Fritillaria and Trichosanthes Fruit Powder (bei mu gua lou san) C:Chuan Bei Mu D:Gua Lou A:Tian Hua Fen, Fu Ling, Ju Hong, Jie Geng Pattern: Dry phlegm in the Lungs Indications: Cough with deep-seated sputum that is difficult to expectorate, wheezing, a dry and sore throat, a red and dry tongue with little coating, and a rapid and thin but strong pulse.

Sargassum Decoction for the Jade Flask (hai zao yu hu tang) Hai Zao, Kun Bu, Zhe Bei Mu, Ban Xia, Du Huo, Chuan Xiong, Dang Gui, Qing Pi, Chen Pi, Lian Qiao, Gan Cao Pattern: Qi goiter from stagnation of dampness, phlegm, qi and blood. Indications: Masses in the center of the neck which are rock-like in hardness, immobile, cause no change in the color of the skin, and do not ulcerate. Tongue has thin greasy coating, pulse is wiry and slippery.

Poria, Cinnamon Twig, Atractylodes Macrocephala, and Licorice Decoction (ling gui zhu gan tang) Fu Ling Gui Zhi Bai Zhu Zhi Gan Cao Pattern: Congested fluids in the epigastrium Indications: Fullness in chest, palpitations, dizziness, S.O.B., watery sputum. Tongue: pale, swollen, Greasy white coating Pulse: wiry, slippery

17. Poria, Licorice, Schisandra, Ginger, and Asarum Decoction (ling gan wu wei jiang xin tang) Gan Jiang, Xi Xin, Fu Ling, Wu Wei Zi, Gan Cao Pattern: cold congested fluids due to yang deficiency and ascendent yin. Indications: Coughing, profuse clear sputum, excess saliva, discomfort in the chest. Tongue: white slippery coat. Pulse: wiry, slippery

18. Three-Seed Decoction to Nourish One's Parents (san zi yang qin tang) Bai Jie Zi, Su Zi, Lai Fu Zi Pattern: Cold in the Lungs with food stagnation. Indications: Coughing and wheezing, copious sputum, focal distention in the chest, loss of appetite, digestive difficulties, a white, greasy tongue coating, and a slippery pulse.

19. *Stop Coughing Powder (zhi sou san) C:Zi Wan, Bai Qian, Bai Bu D: Jie Geng, Chen Pi A:Jing Jie E:Gan Cao Pattern: Lingering cough due to wind attacking the Lungs. Indications: Coughing with slight chills and fever, or no chills and fever, an itchy throat, a thin, white tongue coating, and a moderate, floating pulse.

20. This is wind attacking the Lungs. This presentation usually occurs as the sequela to an externally- contracted disease that has been treated, but in which the cough persists. CAUTIONS AND CONTRAINDICATIONS: This formula has a tendency to dry and should therefore not be used in treating coughs associated with yin deficiency. It was designed for treating wind-cold and should not be used without modification in treating coughs due to heat in the Lungs.

21. *Pinellia, Atractylodes, & Gastrodia Decoction (ban xia bai zhu tian ma tang) C:Ban Xia, Tian Ma D:Bai Zhu A:Fu Ling, Chen Pi, Sheng Jiang, Da Zao E:Gan Cao Pattern: Upward disturbance of Wind-Phlegm when dampness accumulates due to Spleen deficiency Indications: Dizziness or vertigo (possibly severe), headache, a stifling sensation in the chest, nausea or vomiting, copious sputum, a white, greasy tongue coating, and a wiry, slippery pulse.

22. CAUTIONS AND CONTRAINDICATIONS: Contraindicated for vertigo from either ascendant Liver yang or blood deficiency.

24. *Eliminate Wind Powder from True Lineage (xiao feng san) C:Jing Jie, Fang Feng, Niu Bang Zi, Chan Tui D:Cang Zhu, Ku Shen, Mu Tong, Shi Gao, Zhi Mu A:Sheng Di Huang, Dang Gui, Hei Zhi Ma E:Gan Cao Pattern: Wind-heat or wind-dampness with underlying damp-heat. Indications: Weepy, itchy, red skin lesions over a large part of the body, a yellow or white tongue coating, and a forceful, floating, and rapid pulse.

25. CAUTIONS AND CONTRAINDICATIONS: Because this formula is primarily dispersing in nature, it should not be used in cases with marked qi deficiency or blood deficiency.

26. *Minor Invigorate the Collaterals Special Pill (xiao huo luo dan) C: Zhi Cao Wu, Fu Zi, Tian Nan Xing, Mo Yao, Ru Xiang, Di Long Pattern: Wind-stroke or Wind-cold-damp painful obstruction Indications: Chronic pain, weakness, and numbness (especially in the lower extremities) due to wind-stroke. Also for fixed or migrating pain in the bones and joints with reduced range of motion due to wind-cold-damp painful obstruction. In both conditions, the symptoms are aggravated by cold. The tongue coating is white and moist.

27. Lead to Symmetry Powder (qian zheng san) C:Bai Fu Zi Jiang Can, Quan Xie Pattern: Channel stroke of head and face, d/t Wind Phlegm, possibly with underlying Deficiency of Wei Qi. Indications: Sudden facial paralysis with deviation of the eyes and mouth and facial muscle twitch. CAUTIONS AND CONTRAINDICATIONS: Contraindicated during pregnancy. Avoid using for long periods of time, due to toxic ingredients. Warming, best used for cold-damp type Wind-Phlegm.

28. Sedate the Liver and Extinguish Wind Decoction (zhen gan xi feng tang) C: Huai Niu Xi Dai Zhe Shi, Long Gu, Mu Li, Gui Ban, Xuan Shen, Tian Men Dong, Bai Shao, Yin Chen Hao, Chuan Lian Zi, Mai Ya, Gan Cao Pattern: Liver and Kidney yin deficiency with ascendant Liver yang Indications: Dizziness, vertigo, a feeling of distention in the eyes, tinnitus, feverish sensation in the head, headache, irritability, flushed face (as if intoxicated), and a wiry, long, and forceful pulse.

29. There may also be frequent belching, progressive motor dysfunction of the body or development of facial asymmetry that occurs over a period of a few hours to a few days, severe dizziness and vertigo, sudden loss of consciousness, mental confusion with moments of lucidity, and an inability to fully recover after loss of consciousness.

30. *Gastrodia and Uncaria Decoction (tian ma gou teng yin) C:Tian Ma, Gou Teng, Shi Jue Ming D:Zhi Zi, Huang Qin A:Yi Mu Cao, Chuan Niu Xi, Du Zhong, Sang Ji Sheng, Ye Jiao Teng, Fu Shen Pattern: Ascendant Liver yang with internal movement of Liver wind Indications: Headache, dizziness, vertigo, tinnitus, blurred vision, a sensation of heat rushing to the head, insomnia with dream-disturbed sleep, a red tongue, and a wiry, rapid pulse. In severe cases, there may also be numbness, twitching and spasms in the extremities, or hemiplegia.

31. Antelope Horn and Uncaria Decoction (ling jiao gou teng tang) C:Ling Yang Jiao, Gou Teng D:Sang Ye, Ju Hua A:Bai Shao, Sheng Di Huang, Chuan Bei Mu, Zhu Ru, Fu Shen E:Gan Cao Pattern: Liver Excess Heat stirring Wind Indications: Persistent high fever, irritability, restlessness, dizziness, vertigo, twitching and spasms of the extremities, a deep-red, dry or burnt tongue with prickles, and a wiry rapid pulse. In severe cases there may also be impaired or actual loss of consciousness.

Also for coma d/t wind-stroke or childhood convulsions with similar presentation, and for stiffness of the tongue and frigid extremities. CAUTIONS AND CONTRAINDICATIONS: Contraindicated during pregnancy. Avoid long-term use (toxic).

Indications: (1) sudden collapse, loss of consciousness with clenched jaw (windstroke); (2) impending loss of consciousness with sensation of fullness, pain, and cold in chest and abdomen; (3) abdominal pain and focal distention in chest with urge to vomit or defecate without doing either, which in severe cases may lead to sudden unconsciousness or coma. All of the above are accompanied by a pale complexion, purple lips, excessive mucus and saliva, cold extremities, a pale tongue with a slippery greasy coating and a submerged, slippery pulse. Caution in weak patients.

Vaporize Phlegm Pill (gun tan wan) C:Duan Meng Shi (Lapis**) Da Huang, Huang Qin, Chen Xiang Pattern: Phlegm-Fire obstructing the orifices, chest or channels Indications: Mania-withdrawal and palpitations with anxiety; or severe, continuous palpitations whichcan lead to coma; or coughing and wheezing with thick, viscous sputum; or focal distension and stifling sensation in the chest and epigastrium; or dizziness, vertigo or tinnitus.

39. In each of these cases there is constipation, a yellow thick and greasy tongue coating, and a slippery rapid and forceful pulse. There may also be facial tics, insomnia, or very strange dreams, sudden, deep pain in the joints that is difficult to describe, nodules in the neck, or a choking sensation. **Lapis Micae seu Chloriti is sweet, salty, and neutral; it scours Phlegm, directs Qi downward, calms wheezing, and stops palpitations. CAUTIONS AND CONTRAINDICATIONS: Contraindicated during pregnancy and post partum. Caution in weak patients. Do not use in the absence of phlegm and heat

41. *Preserve Harmony Pill (bao he wan) C:Shan Zha D:Shen Qu, Lai Fu Zi A:Ban Xia, Chen Pi, Fu Ling, Lian Qiao, Mai Ya Pattern: Food Stagnation Indications: Focal distention and fullness in the chest and epigastrium, abdominal distention with occasional pain, rotten-smelling belching, acid regurgitation, nausea and vomiting, aversion to food, a yellow, greasy tongue coating, and a slippery pulse. There may also be diarrhea.

42. CAUTIONS AND CONTRAINDICATIONS: Unless modified, this formula is contraindicated in cases with Spleen deficiency.
