New Corporate Plan 2015–2017 Annual service and business plans will detail how goals are being achieved – status updates Structural Reform How do we embed H&W reform into CHO and HG structures and priorities? Head of Health and Wellbeing will be appointed in each CHO Each HG will identify a HI Lead at senior level on the hospital group management team Each CHO & HG will publish a HI Implementation Plan Hospital specific plans H Improving Health and Wellbeing in Ireland

Structural Reform (contd) HGs, CHOs, NAS etc. proactively build their own internal capability to respond to the HI agenda, to drive change in management and clinical practice and report on agreed outcomes The Health and Wellbeing Division will continue to strengthen senior management team capacity to reconfigure its services, consolidate resources and develop the Division in the context of a commissioning framework and empowered, accountable health delivery systems Improving Health and Wellbeing in Ireland

Healthy Ireland Framework Action 2.2 ‘Local health partners will engage with local authorities in their work to address local and community development, with the aim of co-ordinating actions and improving information-sharing for improved health and wellbeing’

Local Authorities have a history of improving population health and the new LCDC structures present a further opportunity for joint working and engagement HSE is a key stakeholder in community development and LECP planning process – opportunity for joined up planning across shared priorities including improved Health and Wellbeing 2015 H&WB Operational Plan includes commitment for the co-ordination of Healthy Ireland input to the LCDCs HSE representation on nearly all of 33 LCDCs.
