4 U.S. Department of Veterans Affairs Veterans Health Administration An Amazing First Year! Served approximately 21,000 Veterans and a total of 35,363 participants Significant impact on Veteran families with 8,826 children assisted Of the 21,393 Veteran participants, 3,285 are women (15.4 percent of Veterans served). 3,335 Veteran participants are Operation Enduring Freedom /Operation Iraqi Freedom /Operation New Dawn (OEF/OIF/OND) (15.6 percent of Veterans served) Approximately 85 percent of those discharged from SSVF exited into permanent housing An additional 9.4 percent (1,952 participants) are off the streets in temporary or institutional settings Average Length of Stay for SSVF participants is 93 days Preliminary Data – Not for Release Probability of recommending another Veteran or friend in need to SSVF provider 87.10% Definitely Satisfaction of overall services received from SSVF provider 20.80%71.10% SatisfiedVery Satisfied

6 U.S. Department of Veterans Affairs Veterans Health Administration Monthly Reporting Requirements Submit to VA Repository Data Upload – information on the monthly upload to the VA repository was provided during the Post-Award Conference Repository GTD is from the beginning of SSVF for your agency Submit to Regional Coordinator Dashboard Report HPRP APR for the reporting period (i.e., 6/1/13 – 6/30/13) HPRP APR for year-to-date (i.e., 10/1/12 – 6/30/13) All monthly reports are due by the 5 th business day of the month. Please note the importance of timely and accurate submissions!

7 U.S. Department of Veterans Affairs Veterans Health Administration Program Office Updates SSVF Regional Meetings Presumptive Eligibility – this is important! –Grantees may offer “presumptive eligibility” for those Veterans who appear to meet program requirements, but still need to procure all documents. –Self-certification of Veteran status allowable for a small period of time to avoid delays in service delivery –Such presumptive eligibility will allow grantees to begin case management services (no TFA provision) until official eligibility confirmation. –Written guidance to come from Program Office Grantee Meet and Greet Outcomes Reminder: TFA Breakdown in HHS (TFAR and TFAP) Quarterly Report Templates and Application Resolutions Use of SSVF with GPD TIP Programs
