1 Us Case 5 Delivery Coordination with Vital Records Update, Hearing Screening & Quality Monitoring to Aid Early Pediatric Care Care Theme: Maternal & Newborn Health Use Case 2 Interoperability Showcase In collaboration with IHE

2 Primary Goal : To demonstrate the ability to share patient information across multiple care provider systems, including public/government systems as a means of providing quality care statistics, and to develop long-term care plans, as necessary. Key Points: The demonstration will use multiple IHE profiles to demonstrate the exchange of medical summary information between electronic medical record (EMR) Systems and the development of long-term, quality health care plans based upon state /government historical information. Meaningful Use (MU) Relevance : Improving Quality, Safety, Efficiency, and Reducing Health Disparities Clinical Workflow: 1.A mother is seen by a prenatal care provider who provides antepartum documents to the L&D unit at the Birthing Facility. The mother delivers the child at this facility. A labor and delivery summary (LDS) is completed and made available for the care team of the newborn. An electronic birth registration to the State/Jurisdiction Vital Records with support from CDC-hosted value sets to pre-populate the forms is also initiated. The LDS document is used to pre-populate the Maternal & Child Health-Birth & Fetal Death Report (MCH-BFDrpt) for public health with the selected prenatal care and L&D information. 2.Due to complications, the newborn is transferred to a Specialty NICU. A newborn discharge summary (NDS) is shared with this NICU to facilitate care. At the Specialty NICU, the newborn is placed on monitoring devices and a ventilator due to its medical condition. Hearing screening is performed and results are sent to the Public Health Early Hearing Screening Info System (EHDI-IS) using forms provided by a public health forms service. A newborn discharge summary is provided to the pediatrician in preparation for the initial visit. 3.The EHDI IS system receives hearing screening results indicating that the newborn did not pass the screening test and generates an EHDI care plan to direct the newborn to be referred to follow-up for confirmation and diagnosis, and makes this care plan available through the HIE to the pediatrician. The Public Health Entity develops, publishes and monitors EHDI electronic Quality Measures (e- Measures). 4.The pediatrician retrieves the NDS and the EHDI Care Plan to complete the newborn plan of care. Having this EHDI info so timely, referrals for confirmatory testing of hearing loss and discussion with parents about any risk factors for delayed or progressive loss are facilitated. 5.The CDC/ National Center for Health Statistics (NCHS) receives birth certificate information from all vital records jurisdictions. Statistics are collected and analyzed to monitor the demographic and health status of the population and for epidemiologic research. The Patient visits her OB/GYN pre-delivery at the OB/GYN office which uses an EMR, is then referred to the hospital. 2 & 3a The patient goes to the hospital L& D unit, which uses an EHR system and is admitted. The health plan is sent an admission message and birth event reporting initiated. 3b. Due to complications, the newborn is transferred to a Specialty NICU. A newborn discharge summary (NDS) is shared with this NICU to facilitate care. The newborn is placed on monitoring devices and a ventilator. 3c. Hearing screening is performed and results are sent to EHDI-IS. The patient is discharged. 4. The EHDI-IS system generates an EHDI care plan to direct the newborn to be referred to follow-up for confirmation and diagnosis, and makes this care plan available through the HIE to the pediatrician. 5. The pediatrician retrieves the NDS and the EHDI Care Plan to complete the newborn plan of care. 6. The CDC/NCHS receives birth certificate information from all vital records jurisdictions. Statistics are collected and analyzed to monitor demographics and for epidemiologic research.
