2 Project Proposal To whom it may concern, As part of the Health Services class at Liberty High School, we are required to perform a senior service project involving the community. Our group chose to involve and incorporate Avameer’s retirement center into our project by producing a Health Plan specifically designed to help the elderly maintain a healthy and active lifestyle. We will provide several different educational classes in the following areas: nutrition, fitness and memory enhancement. We recognize the importance of maintaining good health and fitness, no matter what age you are and would find it extremely rewarding to be able to contribute and give back to the elderly community. This project will be challenging, but it is our goal to educate, engage, and to help motivate. We understand that not every resident will want to be involved. However it is our goal to be able to reach out to those who wish to make a change. This project connects to our Health Services Program of Study because it heavily involves the interest in the importance of good health and wellbeing. There will be several different peers involved in this project that organize the different groups. It is important that as younger individuals we try to produce an encouraging environment with lively vibes around the elderly in order to help motivate and engage interest. 3 Proposal Our group chose to involve and incorporate Avameer’s retirement center into our project by producing a Health Plan specifically designed to help the elderly maintain a healthy and active lifestyle. JANUARY – “Nutrition Class” FEBRUARY – “Exercise Class” MARCH – “Memory Enhancement”

4 Current Progress/Update Project proposal presented to supervisor, introduction of plans and ideas Introduced our project, members, and the class goals to the residents Individually and privately quizzed each resident on their level of ability for the following: math, being able to tell/read time, answering word problems, etc. This was done so that we could be aware of what the specific capabilities were of each resident, allowing us to mold our lesson plans to help accommodate.

5 Nutrition Class - Outline Day 1 (Introduction Day) - Introduction to goals (eat a certain amount of fruits and veggies and drink a certain amount of water - Hand out logs (their weekly “homework”) - Discuss the menu options for the week (which option was better/possible alternatives) Day 2 (Smoothie Day) - Give a 15 minutes lesson on the nutritional value of the ingredients we were using in the smoothie being made, as well as the nutritional value of some other possible ingredients to use in smoothies. - Made a soy, peach and berry (raspberries, blackberries, blueberries, strawberries) smoothie for the seniors in the class - Collected the logs and handed out new ones/ go over menus Day 3 (Portion Size/Iron and Fiber Day) - Facts and benefits of getting iron and fiber in their meals - Talk about the right portion size of each food group (show them how to use other objects to get the right amount) - Collect logs and hand out new ones/go over menus Day 4 (Benefits/Consequences Day) - Discuss the consequences of not having a healthy diet (examples of potential health problems) - Overall Benefits of eating a healthy diet (improves cognitive and memory skills) - Last collection of logs (show each senior their progress from the last 4 weeks) - Go over menus

6 Benefits - Nutrition Class Discover the proposed amount of each food group that each senior is supposed receive every day. Each are getting a better sense of how to use the resources that are provided to them. Getting an understanding on how to decide on the “healthier” meal. Able to create new friendships with others in the class. Each senior will be able to know how to determine the right portions of each food group using other objects.

7 January’s Nutrition Class – Current For the first class - Residents were split into groups of two and received a lecture on the importance of proper water intake and healthy food choices. Residents were given their first log: This was so that they could record their water intake and the types of foods consumed that week. For the second class – Residents were shown how to make a simple and healthy smoothie. We showed them dairy free alternatives and how making/drinking smoothies can be an easy alternative to get your 1 cup of fruits and vegetables for the day.

8 Memory Enhancement Lesson The purpose of this class is to enhance the memory of the elderly people. By providing activities, the elderly will work the pre frontal cortex which will improve their memory One very beneficial way of improving memory and cognitive skills is by learning new things, therefore we plan to teach the elderly how to use IPad. We will also teach them how to use the Wii, which really help with cognition since it involves the brain as well as the body Luminosity along with other computerized brain games will be played during these lessons to work the brain.

9 The memory enhancement lesson will last four weeks, every Tuesday at 4pm for an hour. We will meet up around 2:30 to prep for the class, as well as pass out reminders that there is class, hence these people do have memory problems. Ironic Huh. This class will not only provide information on tips to improve memory, it will teach them how to use their resources at Avamere. Our goal is to improve their memory during the class as well as after the class. Mainly by making memory activities habits that they can add to their everyday schedule. If the follow up with the cognitive lessons taught they will have less of a chance of getting dementia or any memory disorder
