33 APPLICATION - 2011 All of the following documents must be submitted to complete the application process: 2011 Application Packet with supporting documents - (see description with each criteria) A copy of applicant’s ECC ID Card AND Driver’s License (must be a good copy - clear and easy to read) Three Letters of Recommendation to the program (must be written within 3 years from the date of application) Character references must be on letterhead stationary and/or attached business card of the person writing the recommendation letter. Letter must include a signature with the writer’s contact information. - Professional or Business references – not personal (family or friends) Letters may be from an an employer, supervisor, professor with a lab class, or counselor.

34 Application Instructions – WHEN YOU HAVE COMPLETED THE COUREWORK: Make an appointment to meet with one of the Health Sciences counselors (Janice Ishikawa, Chris Jeffries, Rene Lozano or Kelsy Iino) to have your coursework and GPA evaluated using the new forms from the website. 39 SPECIAL NOTE REGARDING A.R.R.T. CERTIFICATION ELIGIBILITY (last page of handout) Students must comply with the “Rules of Ethics” contained ARRT Standard of Ethics. The A.R.R.T. may deny applications for certification if the applicant has been found guilty of dishonesty, fraud, deceit, acts which deceive the public, felony, child abuse, sex offender crimes; acts involving narcotics, dangerous drugs or devices, assault and/or battery and other crimes.
