1 Child and Family Outcomes Chapter 6: Child Outcomes Summary Form Entry Information

2 Instructions for Completing the COSF Complete demographic data - Questions 1-5 1. Date Completed: Enter the date that the IFSP team (or individual team members) completed the Child Outcome Summary Form (mm/dd/yyyy). 2. Child’s Name: Enter the child’s first and last name as the name appears on the child’s EIP record and in KIDS. 3. Child’s Date of Birth: Enter the child’s date of birth (mm/dd/yyyy). 4. Child’s sex: Check male or female to record the child’s sex. 5. County/borough of Residence: Enter the FIPS code from the list of FIPS codes provided for the child’s county of residence.

4 Instructions for Completing the COSF IFSP Team Members – Question 6 6. Check the IFSP team members who completed the Child Outcomes Summary form. o If IFSP team members agree on the child’s status, one form should be completed and ALL appropriate boxes checked. o If IFSP team members have different views about the child’s current status, each IFSP team member can complete a separate form.

6 Instructions for Completing the COSF Functional Child Outcomes - Questions 7a, 7b, and 7c The Child Outcomes Summary Form asks you to think about and report on how the child behaves across many different settings and situations, and with different people in three functional outcome areas. For each of the three summary questions, the IFSP team needs to decide the extent to which the child displays behavior and skills EXPECTED FOR HIS OR HER AGE related to each outcome area.

7 Instructions for Completing COSF Definitions for the ratings - Questions 7a, 7b, and 7c Overall age-appropriateCompletely means:7Child shows functioning expected for his or her age in almost all everyday situations that are part of the child’s life. Functioning is considered appropriate for his or her age. No one has any concerns about the child’s functioning in this outcome area. 6Child’s functioning generally is considered appropriate for his or her age but there are some concerns about the child’s functioning in this outcome area. Overall not age- appropriate Somewhat means:5Child shows functioning for his or her age some of the time and/or in some situations. Child’s functioning is a mix of age appropriate and not appropriate functioning. Functioning might be described as like that of a slightly younger child. 4Between somewhat and emerging Emerging means:3Child does not yet show functioning expected of his or her age in any situation. Child’s behaviors and skills include immediate functional skills upon which to build age appropriate functioning. Functioning might be described like that of a younger child. 2Between emerging and not yet Not yet means:1Child does not yet show functioning expected of a child his or her age in any situation. Child’s skills and behaviors also do not yet include immediate foundational skills upon which to build age-appropriate functioning. Child’s functioning might be described as that of a much younger child.

9 Instructions for Completing the COSF Types of Supporting Evidence - Question 8 Ratings are expected to take into account the child’s functioning across a full range of situations and settings. Many types of information could be considered in selecting a rating. For each of the three functional outcome areas, the types of supporting evidence used to help the team decide how to rate the child’s current status needs to be identified. Please check ALL types of supporting evidence used by the IFSP team to rate the child.

11 Instructions for Completing the COSF Developmental Tests - Question 9 List any developmental tests that were conducted for the child and used by the IFSP team members in deciding how to rate the child’s current status in each of the three functional outcome areas, by using the test code from information binder. If tests were used that are not included in the list, please write in the name and edition of the test in the space provided. Do not include any tests in the 2nd section of the table that were previously recorded using an assigned test code.
