Purpose of the Course For each internship credit a student sets measurable career performance and learning objectives that go beyond the present scope of his/her job. The student then works to accomplish those objectives during the semester. Work Hours Requirement The Formula: For every 75 hours of employment in a semester a student may earn 1 credit hour of co-op/internship. Student Checklist __________________________________________________________________ Complete by 15 May 2015 Attend an Internship Orientation Meeting (three sections are scheduled during the first full week of semester). Complete online UVU Orientation questionnaire, located at http://www.uvu.edu/internships/students/orientation.htmlhttp://www.uvu.edu/internships/students/orientation.html Meet or email the Internship Manager to confirm your enrollment and qualifications. Email your Internship Manager with your project proposal. (All Internship students are required to use the UVLink email address (uvid#@uvlink.uvu.edu) to facilitate communication with the Internship Manager).uvid#@uvlink.uvu.edu Request an electronic packet from your Internship Manager or access it from the FTP server. ftp://cseftp.tc.uvu.edu/CNS/orchard/ftp://cseftp.tc.uvu.edu/CNS/orchard/ Deliver a copy of the form letter to your supervisor at work. Solicit his/her assistance in developing your objectives. Your supervisor must initial the Objectives Worksheet that he/she has received this letter. Complete the Student Information Sheet and email it to the Internship Manager. 22 May 2015 Work with your work supervisor to develop appropriate objectives for the semester. Coordinate with Internship Manager to ensure Objectives Worksheet meets requirements. Submit typed signed objectives in final form to your Internship Manager. Your supervisor must initial the Objectives Worksheet that he/she has received the form letter. 21 May 2015: Last Date to drop without receiving a W 27 May 2015: Scholarship applications due 19 Jun 2015: Submit completed Mid-term Evaluation Form to your Internship Manager. If requested, assist in coordinating a job site visit between your work supervisor and your Internship Manager. 31 Jul 2015: Complete your term paper and the Final Evaluation Form. Submit to your work supervisor for approval. 07 Aug 2015: Submit term paper, Final Evaluation Form, and End of Course Survey to Internship Manager. 14 Aug 2015: All required documents must be submitted to receive final grade.
