2 Overview: New tool that helps coaches and parents to recognize the signs/symptoms of a concussion and to respond quickly and appropriately to this potentially serious medical situation. Allows a coach/parent to record pertinent information regarding a child with a suspected concussion and share that information via email with doctors or other medical professionals. Provides an easy way to monitor recovery and determine a young athlete’s readiness to return to play. This tool is an invaluable way to learn about how concussions are diagnosed and treated.

3 Main Menu: New Incident: report a new incident and determine likeliness of a concussion. Home Symptom Monitoring: Monitor youth over course of time. Concussion Info: Learn more about concussions and issues important to coaches and parents. ACE Post-Concussion: A guide supplied by CNMC regarding the main points of treating a concussed child at home Return-to-Play Guide: Provides guidelines for exercise to ensure an athlete is ready to return to play.

4 Practice / Actual Injury : Allows the user to test using the app without impacting the data collection Users can familiarize themselves with the many features/functions

5 Signs and Symptoms: Report signs that the athlete is showing to indicate how they look to be performing. Report symptoms that they say they feel. Excellent for keeping a record of an athlete’s status after an incident

6 Danger Signs: Stay aware of the possible danger signs that could impact a youth’s well-being. Report symptoms that they say they feel. Excellent for keeping a record of an athlete’s status after an incident

7 Final Profile View: Summary: This summary provides a detailed list of what questions were positively endorsed. Guidance: The summary also provides the next steps that should be followed to prevent further injury. Share: The details of the incident can be shared via email to doctors, parents, trainers, etc. Location: Show the GPS location of the incident for later reference.

8 Return-to-Play Guide: Guidance: Provide coaches and parents with a step by step guide to returning a youth to play. Safety: Assists tiered testing of athlete’s ability prior to return-to- play

9 Home Symptom Monitoring: Summary: This summary provides a detailed list of which signs or symptoms in which areas were positively endorsed. Progress: See how the athlete’s condition is improving over time. Share: The details of the assessments can be shared via email to doctors, etc.

10 Concussion FAQ’s: General: This portion of the FAQ’s provides a general overview of concussions. For Coaches: focuses on the issues that coaches care about regarding concussed athletes. For Parents: focuses on the issues parents are most concerned with regarding their concussed child.

11 ACE Post-Concussion Home/School Instructions: Provides guidelines for parents or school personnel regarding a concussed child or student. Created and distributed by Children’s National Medical Center for guidance to their patients who have been concussed. This document can be shared via email with other parents, coaches, or school officials.

12 Data collection for research: Assist in research by downloading the CRR and opting-in to data collection. Data is de-identified to protect the anonymity of those being assessed. Research organizations with a stake in the outcome of athletes will be given access to further research.

13 Giving back to research: 15% of all sales go directly back to supporting concussion and TBI research Children’s National Medical Center will receive ½ of the donations. Matthew Gfeller Sport-Related Traumatic Brain Injury Research Center will receive the other ½ of donations. 15%
