A: I t is highly recommended for each participant to have a family member complete the Family Baseline Questionnaire. However, If it is impossible for that participant to find a family member to complete the family questionnaire, you can send the rest of his/her completed baseline forms/questionnaires to the CC and note this circumstance under the comment section of the Tracking Logsheet.
