2 Main Objective: Improve patient health literacy and increase general health screenings for chronic illnesses

3 Accomplishments Scheduled Cancer Screenings Implemented new chronic disease charting system Created Patient Good Health Agreement Modified existing CHP forms Created a Schedule II Drug Prescribing Policy Designed general health screening brochure Assisted Michael Dixon with the facility change of CHP Designed a CHP patient survey Volunteered at the clinic

4 Breast Cancer Screenings GOAL: 100 women ages 40+ no mammogram within past year Received a clinical breast exam (cbe) Free mammogram from Cooper Hospital 4 screening events (June 29, July 29, August 12, August 26) 92 women scheduled 23 attended thus far

5 Prostate Cancer Screening GOAL: 50 Men ages 50+ (minority men 45+) Received a digital rectal exam Referred for blood work (psa) if necessary 2 screening events (July 22, August 5) 62 men scheduled 15 men attended 1 st screening out of 29 scheduled

6 Chronic Disease Charting System Efficient, concentrated information Used in patient files Track overall health Track needed and completed general health screenings

7 Patient Good Health Agreement Maintain good health Follow physician’s recommendations Awareness of CHP’s volunteer staff

8 Schedule II Drug Prescribing Policy As of March 1, 2010, physicians in the state of New Jersey are permitted to prescribe patients a 90 day supply of Schedule II drugs. Reduces weekly patient flow in the waiting room More convenient for the patients

9 General Health Screening Brochure Benefits of health screenings for chronic diseases Contributes to patient health literacy Targets the main chronic illnesses prevalent in Camden, NJ with respect to race Beneficial information for both men and women
