4 The medical assessment form is a supplemental form only The medical assessment was designed for school nursing staff in case additional information was needed for documentation. The medical assessment does not have to be filled out with each I/A form The medical assessment does not replace the I/A form 4

6 To remove a digital signature in Adobe you must delete the signature file from the computer. The file is located in the user’s folder under Document and Settings. Here’s the path to the file: C:\Documents and Settings\username\Application Data\Adobe\Acrobat\10.0\Security Username will be replaced the person’s login name. The file to be deleted is the pfx file. Once the file has been deleted, you will be prompted to create a new digital signature. 6
