St John’s Community Hospital Administration of IV Antibiotics How did the initiative come about? Previously administered in acute hospital settings Trend now to deliver in home or Community Hospitals Result of “Out Patient Parentral Antimicrobial Therapy Clinical Care Programme” No patient receiving IV AB who can be treated out of hospital to remain as an inpatient Pilot programme for administration of IV AB in the community

St John’s Community Hospital Administration of IV Antibiotics How Pilot was Introduced Introduced as part of Joint Implementation Group across the Acute and PCCC Services Meetings held with: –Discharge Liaison and Senior Project Officer –Medical Consultants –Director of Nursing –Service Manager for Older People –Director of Nurse Education Clinical pathway introduced Development of a policy Staff education Sourcing of equipment Follow-up meetings – pilot date determined

St John’s Community Hospital Administration of IV Antibiotics Pilot Programme Pilot Pilot started July 2011 – Nov 2011 Six patients treated within pilot 29 Acute bed days saved Reduction in transport costs Positive Service User experience – “only for I could get IV AB in St. John’s, I’d be dead” – B Anderson 20/11/11 Positive Staff experience No delay in activation of treatment
