14 SAFETY CONTRACT—CHEMISTRY Please read the following statements regarding your behavior in the chemistry laboratory. You are required to sign this document and turn it into your teacher. My teacher has pointed out to my class all of the following safety features in the chemistry laboratory: safety shower, eye wash stations, first aid kit, fire blanket, fire extinguishers and fire exit.

15 I agree to the following safety procedures when I am in the chemistry laboratory: Wear goggles on your eyes at all times during chemistry laboratory experiment, when using any amount of chemicals, or whenever my teacher deems it necessary to wear goggles. An apron is also sometimes required. Conduct yourself in a responsible manner at all times in the laboratory. Follow all written and verbal instructions carefully. If you do not understand a direction or part of a procedure, ask the instructor before proceeding. Perform only those experiments authorized by the instructor. Never do anything in the laboratory that is not called for in the lab procedures or by your instructor. Unauthorized experiments are prohibited.

16 I agree to the following safety procedures when I am in the chemistry laboratory: Never remove chemicals or laboratory equipment from the lab area without permission from your instructor. Never enter the chemical storage room or preparation area unless given specific permission from your instructor. Be prepared for your work in the laboratory. Read all procedures thoroughly. Never fool around in the laboratory. Horseplay, practical jokes, and pranks are dangerous and prohibited. They are reason for not only detention, removal from the lab resulting in a failing grade, but also suspension from school. Keep aisles clear.

17 I agree to the following safety procedures when I am in the chemistry laboratory: Experiments must be personally monitored at all times. You are assigned a laboratory station at which to work. Do not wander around the room, distract others, interfere with the experiments of others, or talk across the room. Contact lenses should not be worn if possible whenever in the laboratory. No coats, sweaters, or loose clothes. No book bags in the lab. You must wear closed toed shoes (keep a pair in your locker). Long hair must be tied back. Report an accident or spill to the instructor no matter how trivial it may appear.

18 I agree to the following safety procedures when I am in the chemistry laboratory: Never handle broken glass with your bare hands. Use a brush and dustpan to clean up broken glass. Place broken glassware in the designated glass disposal container. Examine glassware before each use. Never use chipped or cracked glassware. Always wash dirty glassware.
