13 Resources and Methodology Poverty and Low-Income Population: Census American Community Survey (ACS) Estimates, 2006 – 2010 data by zip code, “Ratio of Income to Poverty Level” Penetration of Low-Income Population: Census ACS and UDS Mapper (www.udsmapper.org), FQHC patients as a proportion of low-income residents, by zip code.www.udsmapper.org Low-Income Population Not Seen by an FQHC: UDS Mapper; mapped by zip code is FQHC patient base by zip code subtracted from low-income population. Look-Alike and “other” patient data not available at zip code level. Health Indicators: – Uninsured Population: ACS uninsured population by county for 2011. Rate of the uninsured by county was multiplied by zip code population to estimate uninsured population by zip code. – Diabetes and Adults Without a Usual Source of Primary Care: Behavioral Risk Factor Surveillance System (BRFSS) data by County for 2009 was gathered from the California Department of Public Health (CDPH) report “Diabetes in California”. Diabetes and medical home rates at the county level were multiplied by the adult population in each zip code (adult population by zip code obtained from ACS 2007 – 2011 estimates) to estimate the adult population for each indicator. – Note: Given the high proportion of low-income residents in TCE’s Healthy Communities, data extrapolated using county rates applied to zip code data likely underestimates the uninsured and diabetic populations, as well as those lacking a medical home. – Low Birth Weight CDPH, Improved Perinatal Outcome Data Reports by County http://ipodr.org/001/vs/2010objectives.html.
