16 Bridging Project Overview New partnership between Administration for Children and Families, Centers for Medicare and Medicaid and the Corporation for National and Community Service. – Engaging Senior Corps Grantees and Community Action Agencies in four states: Arizona, Michigan, Ohio and New Jersey. – In Arizona, the project focus is Maricopa County. – Bridging partners include: Mesa: East Valley Adult Resources (Senior Corps Grantee) and MESACAN Phoenix: Area Agency on Aging, Region One (Senior Corps Grantee) and City of Phoenix Community Action Program

17 Bridging Project Overview Project Focus & Performance Period: 10/1/13-3/31/14 VISTA: One year term serving at Community Action Agency. Member assignment will be to develop systems and supports to assist individuals gain information and access to the new Affordable Care Act insurances and other assistance programs offered by the Action Agency. VISTA member will assist with project data collection and reporting on the project performance measures. Senior Corps: 6-12 month term At least 25 RSVP volunteers will be recruited by the participating Senior Corps programs to serve at the participating Action Agency. RSVP volunteer will complete the training to become Certified Application Counselors to assist individuals gain information, access and to apply to the new Affordable Care Act insurances offered through the Market Place in Mesa and City of Phoenix.
