12  Kindergarten teachers and support staff will be administering the DIAL-4 screening (Motor, Concepts, Language) and letter identification  Nurse will complete vision and hearing screening (you will be notified if your child does NOT pass)  Counselor will meet with your child to get to know him/her  Your child will receive a book courtesy of Republic PTO

13  Check-in table: child will receive name tag, parents will receive parent questionnaire  Child will be escorted to the screening area  Parent/Guardian will be escorted to the enrollment area  Parent/Guardian will need to complete the DIAL-4 Parent Questionnaire FIRST before completing enrollment paperwork

14  Parents will complete full enrollment packet while their child is being screened  Bring proof of residency (utility bill, rental contract, real estate contract); copy of your child’s birth certificate; and copy of current immunization record  Have emergency contact and doctor information available

19  When screening is completed, your child will be brought to you  Parents as Teachers will score the screening and come get you to go over the results  Allow 90 for screening and scoring  Childcare is not provided; we encourage you to final alternative arrangements for siblings

20  Percentile Rank: 16-84 %ile is in the average range OK range 8 percentile and above Potential Delay below 7 percentile  Standard Score: 85-115 is in the average range OK range 77 and above Potential Delay below 76

25  Forms will be available as a resource for families to do over the summer to help develop skills in motor, concepts, language, self-help development, social- emotional development, and other areas. These will be given to parents if a potential delay is noted.
