Yes/No. Total tons produced for the most recent calendar year. 8.Does the facility produce fine stone qualified for use in State DOT asphalt? Yes/ No. Total tons produced for the most recent calendar year. 9.Does the facility produce coarse stone qualified for use in State DOT concrete? Yes/ No. Total tons produced for the most recent calendar year. 10.Does the facility produce fine stone qualified for use in State DOT concrete? Yes/ No. Total tons produced for the most recent calendar year. 11.If the facility was acquired in the past year, the name of the company from which the facility was acquired. 12.Year of acquisition. 13.Is the facility currently operating? Yes/ No
