2. Polypharmacy / Poly-MART Details Polypharmacy Medication Analysis & Report Tool (Poly-MART) was designed to support OTSG Policy 13-032. This tool has been designed to identify Army Active Duty, National Guard (mobilized longer than 31 days), & Reserve (mobilized longer than 31 days) enrolled to an Army installation for direct care. Utilizing this method of populating service member information is dependent of current enrollment information, which can be controlled at MTF enrollment locations. Medication prescription history is current for the given month where the TRICARE pharmaceutical benefit was utilized; including MTF, Mail Order, & Retail Network Data. The ER data is current on a rolling 12 month time frame. Data is captured from the PDTS data warehouse and M2 data repository.

3. Bucket Design The PEC/POC added a bucket numbering convention to correspond with specific circles and overlapping areas. The reports within the tool uses the bucket terminology with numbers to identify which circle or area the members fall in.
