1 Correction Of Disorders Of Microcirculation In The Urinary Bladder Wall For Women With Urination Disorders Introduction Urination disorders are one of the serious problems in modern urology. Introduction of a1– adrenergic blockers into clinical practice made it possible to develop brand new methods of medicamental correction of urination disorders. The purpose of the study was to estimate the dynamics of indices of microcirculation in the mucous membrane of the urinary bladder during the conservative treatment used for women with urinary disorders. Conclusions The positive effect of conservative treatment was noted by 9 (45%) patients of the first group and 16 (80%) patients of the second group. The patients of the second group had a more rapid normalization of clinical manifestations and indices of the basal blood flow as it was shown by the Laser Doppler Flowmetry data. The a1– adrenergic blocker ARTEZIN interrupts the pathogenetic chain connected with the bladder ischemia at the very beginning – at the adrenoreceptor level, which manifests itself in the bladder relaxation during the filling phase and the bladder pouch function enlargement. Results After the treatment the patients of the second group noted such results in the urinary bladder mucous membrane as statistically valid decrease in pulse amplitude and slow oscillations, increase in fast oscillations amplitude, significant improvement of active microcirculation mechanism, and declining rates of passive regulation mechanism as well as vascular tension. According to the urination diary and the questionnaire data, positive effect of the conservative therapy was noted by 9 (45%) patients of the first group and 16 (80%) patients of the second group. Methods We examined 40 women with different urination disorders who had been treated for this reason several times. The patients were aged from 18 to 69 (the average age was 38.7 years old), the length of illness varied from 1 to 20 years. The first group included 20 patients who had complex conservative therapy. The second group included 20 patients who had 2 milligrams of a1– adrenergic blocker ARTEZIN (doxazosin) every night. The treatment period was from 1 till 3 moths. The urination diary was being filled in according to the standard method during 72 hours registering the time and the volume of every urination, as well as the presence of imperative feelings of urination and urinary incontinence cases. Laser Doppler Flowmetry method was used before and after the treatment to study mucous membrane microcirculation of urinary bladder for women with urination disorders.
