8 8 These three measures are included in the Performance Measures Definitions for SFY 2013 document. Each will replace existing measures in SFY 2014. They are being shared now as DMHA will be monitoring performance on the measures quarterly during SFY 2013.

11 Targets for Average Numbers Served will be provider specific and based on actual performance between July 2011 and March 2012 11 Spring Provider Meeting May 2012

12 12 DMHA is open to reviewing target change requests SFY 2013 Target review requests will be accepted for consideration as soon as SFY 2013 targets are distributed. No requests will be accepted after April 1, 2013. If your agency has a review request please email a written request with justification for Consideration for Change of Performance Measure Target to (sue.lummus@fssa.in.gov) Spring Provider Meeting May 2012

13 Review historical data if deemed necessary. The Relationship Management review team will review data summary and request to make a team determination of accepting/denying or recommending an alternative target. 14 Within 10 business days of receiving a Performance Report a provider may email a written request to review performance data to Sue Lummus. The request must provide a detailed description and data which outlines the perceived discrepancy between the provider’s data and the Performance Report.
