Karren History of Emergency Medical Services Emergency care developed during warfare at the beginning of the 20th century. By the 1960s, domestic emergency care lagged behind. Staffed emergency departments were often limited to large urban areas. The local funeral home was the ambulance provider. Now the EMS system allows patient care from the time of injury or illness until rehabilitation or discharge.

Karren Medical Direction and Oversight  EMT’s are designated agents of the medical direction of the EMS System.  The Medical director (Dr. Marvin Wayne) Provides medical oversight.  Legally responsible for clinical and patient care aspect of the EMS system.  Oversees and provides continuing education.  Facilitates the Quality Improvement system.  Provides medical direction by establishing guidelines, or protocols.

Karren Quality Improvement or Continuous Quality Improvement In Whatcom County, there is a CQI committee that is overseen by Dr. Wayne. The committee consists of representatives of every Fire District and some agencies within the county EMS system.

Karren Issues in Patient Safety Safety of care is one of the most important issues High-risk activities Transfer of Care Poor communication Carrying/moving patients in a risky manner. Involvement in an ambulance crash. Lack of spinal immobilization or improper spinal immobilization procedure. Steps to prevent errors during patient care Develop clear protocols Light the scene effectively. Minimize interruptions during assessment and emergency care. Clearly mark all drugs and packages. Reflect on actions. Question all assumptions. Use decision aids if necessary. Ask for help if needed.

Karren Research and EMS Care Medical Practice has evolved to evidence-based medicine. Evidence-based medicine focuses on clear evidence that procedures, medications, and equipment improve patient’s outcome. Evidence-based decision making;  Formulate a question about emergency care.  Search medical literature for validity and reliability.  Appraise the evidence for validity and reliability.  Change protocol if evidence supports a change in practice. Initiatives are underway to increase the amount of research conducted to support or change delivery of prehospital emergency care.
