13 How and When Should Schools Report Bank Changes? Bank Changes should be submitted to the Comptroller’s Office approximately one month prior to the requested effective date. Requests must be in writing, and can be submitted by completing and faxing a Banking Information Change Form to (717) 787-3593. A form can be obtained from the website www.pde.state.pa.us/school_acct
