3 SanDisk Confidential 3 CMDDS Sheet Input Area highlighted in yellow-General information is mandatory for CM to disclose Our P/N: SanDisk part number. Part Name: Must same as SanDisk part number Mass: Product total weight. ( note: no need to fill manually, check input macro will calculate ) Contact information: CM contact information including CM name, CM code (received from SanDisk procurement/leave it as blank), contact name, address, email and phone number etc.. Request ID: Must input as “SanDisk”.

5 SanDisk Confidential 5 CMDDS Sheet Input Detail homogenous material information are mandatory for CM to disclose Used Region: Location where homogenous subcomponent used in product. Material Name: Material type used. Homogenous Component/Subcomponent: Subcomponents used in homogenous material. Element Name/Chemical Substance: The chemical elements included in homogenous subcomponent. Purpose of Containment: For what purpose substance is used.

6 SanDisk Confidential 6 CMDDS Sheet Input Weight: Total Homogeneous subcomponent weight and substances total weight need match and input manually. CAS#: Chemical Abstracts Service Registry Number, which is a unique identifier for the chemical substance. CAS# should match with respective substance. Make sure substances CAS# is not included in Controlled and Reportable substances CAS# list (ref to GP). Note: Homogenous PPM level is not required to input manually. Total homogenous PPM level of each homogeneous subcomponent should be 1 million. Substance PPM=(substance weight/homogeneous subcomponent weight)*1000000

9 SanDisk Confidential 9 Lab Test Data Sheet Input General Information: General information will be captured from MDDS Form sheet by macro process The 3 rd Lab Test Result Test result: On the whole, input data per test result in lab test report; If test result is non detected, input “ND”; if CM could not provide any test report, input test result as “N/A”. Test date: Disclose the report publish date as test date. If there is no publish date, disclose latest date in testing period. RoHS Exemption: Select RoHS exemption based on dropdown menu. If there is no exemption, leave it as blank.

11 SanDisk Confidential 11 SanDisk only accepts the lab test reports from ISO17025 certified 3rd party laboratories. Content disclosed in completed lab test report from a 3 rd party laboratory: 1. General information: serial number of test report, supplier name, sample name, sample description, sample type, test period and so on. 2. Testing process and test result: test requirement, test method, sample preparation, test substance, MDL and test result. 3. Sample picture. MSDS can’t be accepted as a test report to SanDisk. 1/10/2015 Note for Lab Test Report

12 SanDisk Confidential 12 Incorrect lab test report example: Example 1: The description for ink in lab test reports are mismatched.  All the lab test reports description for ink should be consistent. Example 2: The color description in MDDS mismatch with description in lab test report. Especially ink, power, solder paste etc. 1/10/2015 Incorrect Lab Test Report Example

15 SanDisk Confidential 15 BOM and Material Weight Sheet Input Completely disclose the BOM materials used in the BOM spread sheet. Memory wafer, Controller wafer, Die attach film and Gold wire material calculations are to be disclosed at the material weight spread sheet. 16 SanDisk Confidential 16 Macros Usage Sequence Clear Format ( Do not forget to delete attachment in CMDDS ) Data Input in CMDDS Sheet Check Input in CMDDS Sheet Data Input in Lab Test Data Sheet Attach CoC and Lab Test Report in Lab Test Data Sheet Check Input in Lab Test Data sheet Export Data in CMDDS Macros are used to check data calculation, data integrity and CMDDS format accuracy automatically after CM input data.

17 SanDisk Confidential 17 Macros Usage Clear Format: Before data input to MDDS, click “Clear Format” to delete unnecessary data or record. Note: CM shall delete attachments lab test report manually, as “Clear Format” function could only clean words; attachments are not included.

18 SanDisk Confidential 18 Macros Usage Check Input in MDDS Form Sheet: Data Input Manually Disclose homogeneous material information from column A13-H13, J, M is mandatory. Data Calculation Automatically 1. Data Calculation: No need to input any data in Column I, K, L. Macros will remind before calculate automatically if any data filled.

19 SanDisk Confidential 19 Macros Usage After input data from column A to H, J and M, click “check input” in CMDDS sheet, mass(B9), unit element mass, homogenous component/subcomponent rate and content rate will be calculated and shown automatically.

20 SanDisk Confidential 20 Macros Usage 3. Data Capture from CMDDS Sheet to Lab Test Data Sheet: C5, C6, and C7 are equal to CMDDS of B7, B8 and J5 respectively. B13 is equal to CMDDS B7. C13, D13 are equal to CMDDS A13, D13 and below. 2. Data Integrity Verification: 1) PPM level for homogenous subcomponent should be 1 million, macros will remind to modify if PPM level is incorrect. 2) If total homogenous subcomponents weight is equal to total unit element mass, numbers will show green color, else show red color. 3) If total content rate is 100%, number will show green color, else show red color. 21 SanDisk Confidential 21 Macros Usage Check Input in Lab Test Data Sheet: After input data and attach lab test reports, click “check input”, macros will align the format. Note: Make sure all the attachments size are small and do not exceed excel cell, otherwise macros will show errors during “check input”.

22 SanDisk Confidential 22 Macros Usage Export Data: Click “Export” to transfer all the data into MDDS which will be sent to SanDisk. Note: Attachment size may change after export data process, CM need to check and do some adjustment to make it looks normal. Save MDDS in Excel 2003 version.
