19 Social Security Number and Driver’s License You must complete registration with DVS before applying for a Social Security Number or a NC Driver’s License. For Social Security Number:  F-1 students only eligible if approved for employment.  J-1 students eligible without employment. When eligible, students must present to Social Security Administration: 1.Verification letter (visa status/job) issued by DVS 2.Required supporting documents Trips to SSA occur every Wednesday at 10 a.m. Please arrive at International House by 9:45 a.m. For Social Security Number and Driver’s Licenses: WAIT 10 business days after entering the U.S. and registering with DVS before applying. 17

20 Personal Integrity  Always provide truthful responses to Consular, Immigration, DVS, and University representatives  Ensure visa documents have been signed by authorized DVS personnel  Understand your responsibilities outlined in your academic program’s Honor Code NOTE: Honor Code violations can result in forfeiture of your ability to apply for post-degree work authorization and/or cancellation of your F-1 program. 18

23 Final Checklist Almost Done! You now must take the following to the DVS Office Monday through Friday between 8:30 a.m. and 4:30 p.m.: 1. Your ORIGINAL visa documents 2. Your completed SEVIS E-Registration FormSEVIS E-Registration Form 3. Signed Acknowledgement FormAcknowledgement Form 4. J-1 students only, the completed J Health Insurance FormJ Health Insurance Form 5. If you are a non-Duke sponsored J-1 Student (i.e. Fullbright, IIE, AMIDEAST, etc.) you must schedule an appointment with your DVS advisor directly to complete your registration.
