Attending a town hall meeting without presenting DOES NOT COUNT. Include the following in “Short Description”: 1. groups and/or people present 2. topics discussed (e.g. overview of Smithfield’s 2011 Sustainability Goals) No survey entry  no SMS credit. **SMS** Please enter the number of community stakeholder meetings hosted by the facility THIS MONTH. (acceptable meetings include; speaking at the Lions club, Rotary, etc - having any type of open invite to discuss facility programs with the general public such as RMP, or inviting a group on site such as the chamber of commerce, fire department, or school group. These are NOT meetings with employees, stockholders, etc - these are meetings that speak to the public in your local town. 2 per year Stakeholder meeting Count

SMS EventCount **SMS** Did you send out an “Incident Notification” for each qualified SMS event this month? (If NO = 0, if YES = number of events) Qualified SMS events include: HHH, Clean Up, WWMD, FFA, Stakeholder Meeting, Learners to Leaders, and Environmental Project Application Submission). The number of SMS events in your surveys must match the number of “Incident Notifications”. Be sure to provide a short description of the event when reporting via the incident notification system.
