2 Site Coordinator (YOU) Health Source RI Agency Coordinator Collects certification forms and registration forms from site employees. Sends registration forms directly to Board of Elections. Completes Monthly Activity report and sends to the Agency Coordinator. Board of Elections Voter Registration Coordinator Completes Monthly Activity Report that includes data from all sites in the agency, and transmits to the Board of Elections. Responsible for training of agency employees. Tabulates Monthly Activity Reports for all agencies. Processes completed voter registration forms. Supplies blank voter registration forms to agencies upon request.

8 Completed Voter Reg. Forms You must send completed registration forms with packing slip to the BOE within 10 days of completion by your client. This become 5 days if the registration form was signed within 5 days of the deadline to register to vote. Registration forms can be hand-delivered.

10 Completed Voter Reg. Forms 2014 Statewide Elections (deadline is 30 days prior) September 9, 2014 (Primary) November 4, 2014 (General Election)

11 Monthly Activity Report You must send this to the Health Source RI Agency Coordinator with all your site data. Does not need to be notarized.

12 Monthly Activity Report Back of page must contain registration activity detail for each employee. A fillable version of this form is available at http://elections.ri.gov/agencies

13 Summary of Site Coordinator Responsibilities Coordinate the program at your office; Monitor number of forms available; Manage storage of certification forms; Send a monthly report to the Health Source RI Agency Coordinator; Send completed voter registration forms to the Board of Elections; Notify us if another employee will be coordinating the program.

14 Helpful Tips for the Agency/Site Coordinator Plan on sending completed voter registration forms to the BOE at least once per week. If you will be out of the office, make sure a co-worker completes the requirements outlined today.
