P roblem S olving I nnovator Solving Tomorrows Problems Today Overview of how the 5 why automatically generates an FMEA & Control Plans 5 Why - FMEA Relationship. 1 st 2 nd 3 rd 4 th 5 th Why FMEA Interaction 1 st 2 nd 3 rd 4 th 5 th Why FMEA Interaction 1 st 2 nd 3 rd 4 th 5 th FMEA Interaction 1 st 2 nd 3 rd 4 th 5 th 1 st 2 nd 3 rd 4 th 5 th Why Control Plan Interaction 1 st 2 nd 3 rd 4 th 5 th Why Control Plan Interaction

There are 3 different ways to access the FMEA

The Failure Mode is defined as the manner in which the process could potentially fail to meet the Product or Process Requirements. The Failure Effect is defined as the consequences incurred in the event of an actual Failure. A Root Cause can be defined as a Product or Process or Design Weakness that may result in a failure. The Detection Controls are specific actions taken to detect the Failure Mode if it happens. 5 Why=FMEA Relationship. The FUNCTION describes each Process Step in a Process The Process Requirements generally reflect the Operating Conditions that are required for effective Process Control. The Product Requirement generally reflects the Specifications that must be achieved.

1 st Why = Function The data in this column automatically gets pulled through from the 1 st Why / Function 1 st 2 nd 3 rd 4 th 5 th Why FMEA Interaction

The data in this column automatically gets pulled through from the 2 nd Why 1 st Why = Function 2 nd Why = Requirement 1 st 2 nd 3 rd 4 th 5 th Why FMEA Interaction

The data in this column automatically gets pulled through from the 3 rd Why 1 st Why = Function 2 nd Why = Requirement 3 rd Why = Potential Failure Mode & Failure Effect 1 st 2 nd 3 rd 4 th 5 th Why FMEA Interaction

1 st Why = Function 2 nd Why = Requirement 3 rd Why = Potential Failure Mode & Failure Effect 4 th Why = Root Cause & Prevention Methods The data in this column automatically gets pulled through from the 4 th Why 1 st 2 nd 3 rd 4 th 5 th Why FMEA Interaction

The data in this column automatically gets pulled through from the 5 th Why DETECT 1 st Why = Function 2 nd Why = Requirement 3 rd Why = Potential Failure Mode & Failure Effect 4 th Why = Root Cause & Prevention Methods 1 st 2 nd 3 rd 4 th 5 th Why FMEA Interaction

The Data gets pulled through and to create the FMEA To view where the data comes from, the ‘View” button is selected. The View button will then take the user to the sheet to where the data was originally entered. 1 st Why= Function 2 nd Why= Requirements 3 rd Why= Potential Failure Mode & Failure Effect 4 th Why= Root Cause & Prevention Method 5 th Why= Detection Controls & Detection Weakness FMEA Interaction 1 st 2 nd 3 rd 4 th 5 th

Data gets pulled through and entered to create the Control Plans To view where the data comes from, the ‘View” button is selected. The View button will then take the user to the sheet to where the data was originally entered. The fields that do not contain data are editable 1 st 2 nd 3 rd 4 th 5 th Why Control Plan Interaction
