1. Determining The Objectives
Deciding on the depth and scope of the data In general, the nurse needs to collect data on the three categories of community health determinants: people, place, and social system.

2. Defining The Study Population
According to Maglaya (2004), the nurse must identify the population group to be included in the study based on the objectives of the community diagnosis.

3. Determining The Data To Be Collected
According to Nies and McEwan (2011), deciding on a strategy for community assessment depends on the objective for data gathering, the size, and characteristics of the aggregate and the resources available to the nurse.

Comprehensive Needs Assessment Problem-oriented Assessment
The nurse gathers information about the entire community using a systematic process where data is collected regarding all aspects of the community to be able to identify actual and potential health problems (Maurer and Smith, 2009). Problem-oriented Assessment The nurse focuses on a particular aspect of health(Maurer and Smith, 2009).

4. Collecting The Data The community itself is the primary source of data. Data sources are generally grouped into primary and secondary data

Primary Data Observation - It is done through an ocular or windshield survey and/or participant observation. Survey- It is made up of series of questions for systematic collection of information from a sample of individuals or families in a community.

Primary Data Informant interview - It is a purposeful talk with either key informants or ordinary members of the community. Community Forum - It is an open meeting of the members of the community (Lundy and Janes, 2009).

Primary Data Focus Group- It is made up of smaller group, usually 6-12 members only, in which membership is more homogenous.

Secondary Data Registry of vital events-
Act 3753 (Civil Registration Law, Philippine Legislature) established the civil registry system in the Philippines and requires the registration of vital events, such as births, marriages, and deaths. R.A (Local Government Code) assigned the function of civil registration to local governments and mandated the appointment of Local Civil Registrars.

Summary Table Health Program Accomplishments The first column lists exhaustively the indicators of your health service delivery in the BHS. The next column is the “Target” column where you will place, at the start of the year, the targets of your BHS for each Indicator. Under each succeeding monthly columns, record the number being asked that corresponds to each indicator for the month. Under each quarter, write the totals required. Morbidity Diseases On the Summary Table – Monthly Morbidity Diseases, write on the space provided the month, the name of disease and the number of cases

Secondary Data Disease Registries- It is a listing of persons diagnosed with specific type of disease in a defined population.

Secondary Data Census data - It is a periodic governmental enumeration of the population (Merriam-Webster Online Dictionary, 2012a). Batas Pambansa Blg. 72 provides for national census of population and other related data in the Philippines every 10 years (Batasang Pambansa, 1980).

5. Developing The Instrument
Instruments or tools facilitate the nurse’s data gathering activities. The following are the most common instruments that the nurse used in her data collection (Maglaya, 2004): Survey questionnaire Interview Guide Observation checklist

6. Actual Data Gathering According to Maglaya (2004), it is suggested that the nurse must meet the people who will be involved in the data collection. It is necessary also that the instruments would be discussed and analyzed and if necessary , be modified or simplified in consideration with people who have limitations in terms of educational preparation or available time to finish data collection.
