2 Background Snap40 researched and developed in Victoria Hospital, Kirkcaldy Proven to be as effective as gold standard patient monitoring The device has the potential to support multiple Services / Teams within Fife Home identified as a suitable team to test the functionality

3 What is Snap40 Snap40 is a wireless wearable armband worn by a patient which remote monitors: Respiratory rate Pulse rate Oxygen saturation Skin temperature Movement & Activity Posture Wearable blood pressure cuff Alerts to staff if patient deteriorates (pre-defined limits)

4 Home To provide a service for the predominantly over 75 population, which prevents acute hospital admission or facilities earlier discharge where acute hospital admission has taken place 3 teams in Fife – west, central and east The service operates 9am – 5pm Patients can receive up to 3 visits per day by a member of the nursing team to have manual observations taken Geography of patients mean a member of the team could travel over 100 miles a day / 180 hours to visit 1 patient

5 Project Aim Big Aim: To test with up to 20 patients whether Snap40 technology would support and improve the level of care provided by the Hospital at Home team Specifically we aim to: Test whether Snap40 will provide the same results as standard monitoring Measure whether Snap40 will release staff time to care Measure whether deteriorating patients can be identified sooner or whether patients can be discharged quicker from the service

Test of change due to commence 8 January running until 31 March 2019

7 Issues Test of change planned to start in September 2018 but pulled 1 week before commencement Technology not enabled to deliver what we need eHealth support withdrawn (support became chargeable) Executive Board not supportive without Outline Business Case being in place
