The various equipment available to the PST is evolving, and becoming more versatile allowing for treatment of all skin types within the professional treatment Becoming knowledgeable in basic electricity methods is necessary in todays market, and will give you an edge that your clients are seeking Clients are becoming more savvy and are looking for results that they cannot get at home Incorporating the use of galvanic and high frequency into your treatments will allow for increased skin firming, a deep cleansing and germicidal action on the skin, more hydration and deeper product penetration In order to fully understand how these targeted results can be achieved, we must have a thorough understanding of the functions of the skin Link: Let’s start by reviewing the primary function of the skin

skin functions The primary function of the skin is protection:
Physical protection – the cells of the stratum corneum act as a barrier to prevent TEWL and the absorption of harmful substances Chemical protection – the acid mantle acts as a barrier to prevent penetration of bacteria, viruses, and other potential contaminants Undergraduate Instructor: optional to have students do a brief verbal review of the layers and functions of the skin before proceeding. What are the functions of the skin? Protection, heat regulation, absorption, sensation, excretion and secretion. Let’s discuss the skins primary function – protection. The skin provides physical protection from our environment with the hardened cells of the stratum corneum preventing TEWL as well as the introduction of harmful substances. As skin therapists, we need to find a way around this protective feature of the skin to allow for penetration of beneficial ingredients. There are 2 pathways that product penetrates across the barrier of the skin. Transepidermal route whereby the molecule transports transcellular (through the cells) or intercellular (lipid bi-layer). If the ingredient is small and permeable such as vitamins C and E, the molecules will be transported across the skin transcellularly Products such as AHAs will transport through the space between the cells or intercellularly via the lipid bi-layer Appendageal route comprises the transport via the sweat glands and the hair follicles. Larger molecular ingredients such as essential oils, salicylic acid and retinoic acid will penetrate the layers of the skin via the hair follicle or sweat glands. The skin also provides chemical protection from our environment with the acid mantle acting as a barrier and preventing the penetration of bacteria, viruses and other contaminants. This pH of this acidic film ( ) plays an important role in understanding product penetration with the use of galvanic. What does the term pH refer to? pH is the term that is used to describe the potential of hydrogen in any water soluble solution. If the pH is low (acidic) there is a high concentration of hydrogen ions, whereas if the pH is high (alkaline) there is a low concentration of hydrogen ions. Water soluble solutions conduct electricity and contain positive and negative electrical charges. Acidic products will carry a positive charge, and alkaline products carry a negative charge. The skin’s barrier and pH work together to control what moves into and out of the epidermis. The outer layers of the epidermis are acidic in nature (acid mantle) yet become more alkaline in nature as you move towards the stratum germinativum and dermis to match the pH of the blood and internal tissues. This is an important concept when working with galvanic current, as how we are able to penetrate product into the deeper layers of the skin is dependent on the galvanic polarity used and the pH of the product on the skin. What type of current is galvanic current? A direct current Direct current is an even and uninterrupted current possessing polarity (the quality of having two oppositely charged poles, one negative and one positive) that flows in one direction, and is used to create a chemical change in the skin. Galvanic requires two electrodes to (anode and cathode) conduct the flow of electricity through the body and complete the circuit. These electrodes are also referred to as active or inactive. Anode - the electrode connected to the positive terminal of the machine and has a positive charge Cathode - the electrode connected to the negative terminal of the machine and has a negative charge Active electrode is “actively working on the skin” and is usually held by the therapist (roller, mushroom, tweezer, etc.) Passive electrode is usually a metal rod covered with a damp gauze or sponge either placed behind the client’s shoulder, held in their hand or strapped to their wrist/arm So how do these electrodes “move” the product across the layers of the skin? It is best explained using the science of attraction What happens when you place the magnet close to the fridge surface? Yes, Instant attraction. What happens when you place 2 fridge magnets together? Yes, they repel. What does this have to do with galvanic? In galvanic treatments, the polarity of the active electrode either attracts or repels the actively charged ions in the water soluble solution placed on the skin. As opposites attract each other, so are the positive (+) elements in the substance irresistibly attracted to the negative (‑) electrode and vice‑versa. Therefore, the substances are PULLED not PUSHED into the skin and penetration occurs as a result of attraction from an opposite pole somewhere else on the circuit. Galvanic current and water provide the ladder across the skin for active substances. The pH of the product you are using on the skin will help you determine which electrode to use. desincrustation Deep cleansing treatment for oily/congested skin
Softens sebum and keratinized cells helping to remove impactions Effects on the skin: Softens tissues Dilates the follicle Increases blood flow/ sensitivity Emulsifies sebum Attracts negative ions Desincrustation allows a therapist to electrically dilate the follicles aiming to soften compacted sebum within the follicle and is usually done prior to extractions or where the skin needs deep cleansing of oil build up. A simple analogy is to think of this process similarly to a dishwasher where alkaline detergent actually turns the greasy remains into soap and breaks down any protein clinging to the dishes. In DESINCRUSTATION, the active electrode (held by the therapist) would be on the negative polarity and you would have an alkaline (-) solution on the skin. The negative ions in the solution would be attracted towards the positive pole of the inactive electrode (attached to the client). This process is also referred to as ANAPHORESIS For best results, desincrustation is used on the skin for 4-7 minutes or 3-5 for more sensitized clients. Ideally, our objective is to achieve optimum results while ensuring there is no risk of extreme sensitivity. Here are some typical products which are used as desincrustation solutions (which usually contain salt to help break down the sebaceous matter): sodium chloride 10% - common table salt and water – astringent and antiseptic sodium carbonate 5% - soda ash found in sea water and lakes sodium bicarbonate 10% - Baking soda – alkali – this can be quite dehydrating and cause extreme high coloring Scaling fluid – an active solution infused with calming fruit, herb and plant extracts in a soothing botanical base. And lastly we must decide which attachment to use, this may vary according to the type of machine which you use and may include Metal prong / mushroom electrode (single and double usually covered - collects saponified sebum and prevents skin burning) - lines, crevices Tweezers or flat head (usually covered) - desincrustation, smaller areas Ionto mask – gauze dampened with solution is applied to the face and mask applied to the skin for appropriate time according to treatment (rarely used) Go through the relevant attachments and show the correct setup on the machine you have at your treatment center. It’s also a great idea to have the students practice “wrapping” the electrode as you demonstrate. Link: Now let’s look at iontophoresis desincrustation

iontophoresis A treatment used to infuse water soluble substances deep into the skin Effects on the skin: Tightens and firms tissues Constricts the follicle Decreases blood flow/ sensitivity Reduces edema Attracts positive ions The second way galvanic current can be used is iontophoresis. Iontophoresis is a treatment that is used to infuse water soluble products, primarily serums or complexes deeply into the skin, and can be used to treat all skin conditions. In IONTOPHORESIS, the active electrode (held by the therapist) would be on the positive polarity and you would have an acidic (+) solution on the skin. The positive ions in the solution would be attracted toward the negative pole of the inactive electrode (attached to the client). This process is also referred to as CATAPHORESIS. When would you perform iontophoresis? Can be performed immediately after desincrustation and extractions – serum or complex would be applied to the specific area, and therapist will work with the positive polarity to rebalance and acidify the skin Can be done prior to masque or after removing the masque – serum or complex can be applied to the entire face, and therapist will work with the positive polarity to rebalance and acidify the skin Can be performed as a stand alone service For best results iontophoresis should be done for approximately 4-7 minutes on the skin. We must decide which attachment is best suited for the treatment. This will vary according to the type of machine you have in your treatment center and may include: Metal rollers - face and neck Metal ball - for concentrating around the nose, mouth & eyes Metal prong (single and double) - lines, crevices Metal stamp - face and neck Galvanic Mask - entire face Go through the relevant attachments and show the correct setup on the machine you have at your treatment center. The effects of galvanic desincrustation on the skin can be further customized to treat your client’s specific needs by incorporating deep cleansing, antibacterial and anti-inflammatory agents into the treatment. Reveals possible contraindications to service Assists the therapist in making correct treatment choices Ensures that client’s concerns are addressed Before beginning any form of electrical treatments a thorough consultation is essential. An in-depth consultation will reveal any contraindications to your planned course of treatment, and also give you a glimpse into the client’s current regimen and expectations of your services. The consultation also helps you prioritize in order to ensure that the client is happy with their treatment and you have addressed their concerns. When looking to introduce electrical modalities, pay particular attention to answers for the following questions: In the last year, have you been under a Dermatologist or Physician’s care? Do you have metal implants, a pacemaker or body piercing? Have you had chemical peels, microdermabrasion or any resurfacing treatments? Are you pregnant or trying to become pregnant? Metal implants, pins or plates Cuts, abrasions or open sores Infection Asthma High blood pressure Loss of skin sensation Clients wearing jewelry Before commencing a skin treatment incorporating electrical modalities, it is vital to check that the client is not contraindicated. The following is a guideline for some of the contraindications to electrical modalities. If your client has a medical condition and you are unsure whether your client is contraindicated to electrical treatment, ask them to consult with their physician and provide you with a permission note to proceed with the treatment. Point out that these are contraindications to both galvanic and high frequency. Undergraduate Instructor: Asthma is in reference to ozone and the use of high frequency in the treatment. Dental work is considered a precaution for most clients, but beware of excessive dental work that includes metal fillings, metal braces and/or metal implants; do not perform galvanic current on this client. contraindications

Explain that these are precautions for use, not contraindications. Over bony areas the skin is thinner and more sensitive. If the electrode remains stationary it may cause discomfort to the client and a burn may result. 3 milliamps is the highest recommended setting. You may want to advise students to also check with the manufacturer of their machine. Breaking current and re-applying the electrode can shock the client. If the skin dries, a burn may result. The current should feel mild and comfortable. The client may experience a metallic taste in the mouth as the galvanic current and the saliva undergoing a chemical change. galvanic precautions and method of use

Cover passive/inactive electrode with damp sponge/ gauze and place in contact with client Select the polarity for the active electrode (+) or (-) Apply product to the skin (+) or (-) Cover active electrode with dampened gauze/cotton if applicable Make contact with the skin, gradually increasing intensity When working with galvanic there is a specific method used that is recommended to ensure safety and efficacy of the treatment. This is a guideline regarding the method of use for galvanic current. You will always need to check with your manufacture’s guidelines as to their specific recommendations. Always discuss the process with your client prior to service and answer any questions they may have. *optional to demonstrate this on a mannequin head at the front of the classroom as you go through the steps* To ensure safety and efficacy of the treatment: Make sure the gauze is evenly moist and not wet, and it is in complete contact with the client to ensure good conduction of current Make sure the active electrode is completely covered with cotton if using the tweezers so no metal is exposed. Ensure the sponge/cotton pad of the mushroom attachment also has adequate amount of product applied. Explain that rollers are not covered but there must be ample product on the skin. Discuss the use of a gel based masque applied over the top of a serum to aid with slip and glide if performing iontophoresis Turn on the current slowly Increase the current until felt by client. Do not exceed 3 milliamps. Talk to the client about sensation and get feedback on sensations felt. These sensations may include a tingling on the skin and or a metallic taste in their mouth Continue working for 4-7 minutes Always keep the electrodes moving so as not to create a galvanic burn Be aware that the process is not immediate, the thicker and more coarse the skin is, the longer it takes to break down the hydro lipid film. An alternating radio current – the tesla current
Ozone gas is produced by the electrode which is germicidal and healing High frequency applications: Direct method – using the electrode in direct contact with the skin or over gauze Indirect method – the electrode is held by the client and the therapist’s hands are in direct contact with the skin What is high frequency? High frequency uses an alternating current (AC) or sinusoidal current to help oxygenate the skin which in turn will have an antibacterial, germicidal and drying action on the skin. It does not affect muscle stimulation, and has no polarity, so it cannot create chemical changes in the tissues. The high frequency machine can be utilized in two ways, for two types of treatments. The direct method is the most widely used, usually following extractions to have a germicidal effect. The indirect method is not as widely used but is very relaxing to receive and very beneficial for the skin. Boardwork: Draw the flow of current from A to B alternating backwards and forwards in a zig-zag motion B A High frequency may be used on the following skin conditions: Acne‑type lesions for a "sparking" treatment. Oily skins with overall congestion for drying and germicidal effects using gauze. To aid in penetration of a treatment cream/ampule by increasing blood flow in certain areas To assist in massage for an aging skin using the indirect method. papules and pustules Drying – sebum Effluvation – increases skin temperature The benefits of the direct method of high frequency are best suited for an oilier skin, or acneic client. The drying and healing effects can be beneficial for a congested skin, and it can also help with the flaking that occurs with excessive seborrhea. Treatment should last approximately 8-10 minutes for an oily or congested skin, and 5 minutes for a normal – dry skin. Let’s look at the additional effects of Direct High Frequency in more detail. Stimulates surface tissue Generates heat in the subcutaneous Increases blood and lymph flow Does not stimulate motor or sensory nerves Show and tell: ask for a volunteer to demonstrate Here we can place the gauze across their cheek and show the placement of the electrode over the gauze. We can use a stationary 4x4 gauze with a mobile 2x2 gauze over the top, being worked along the skin under the electrode. *You can also turn down the lights and demonstrate the “spark” on your own hand or arm. You may also wish to demonstrate “interference” at this time – you can do so by holding the hand-piece very close to the actual electrode and alternately placing your hand lower on the hand-piece – away from the electrode. You should be able to see a disruption in the current.* Extra teachers notes: Should you be challenged regarding the sensation on the skin with the high frequency current and the statement that the nerves are not stimulated, as most clients feel a sensation, you can answer them in the following manner. The oscillating speed of this alternating current is too fast to stimulate sensory nerves directly. However, don't forget that one of the main effects of this current is effluvation (the increase in skin temperature). What you are experiencing is an indirect effect of increased heat in the tissues - stimulating sensory perception of something happening in the skin.

Stimulates blood and lymph flow Stimulates heat in the tissues Stimulates sebaceous secretion Induces relaxation and sedation We have seen how we are able to incorporate Direct High Frequency into our professional treatments, but this is not the only option available to us with High Frequency current. Indirect High Frequency is performed at the European massage stage during the treatment where no electrode touches the face and the saturator or indifferent electrode is in the client’s hand therefore channeling the current. The current stimulates heat in the tissues and is more apparent with a light pressure during the treatment than a firm pressure. Consequently, the lighter the massage the more blood flow is stimulated. This is opposite to a manual technique, where a firmer pressure is required to increase blood. This action makes this method ideal for any skin condition where we want to increase blood flow, but do not want to use a lot of pressure. High frequency, when used in this manner, stimulates blood flow and increases the activity of the sebaceous glands. This brings fresh oxygenated blood to the tissues as well as increasing lymphatic activity, warming and nourishing the skin without pulling or stretching. This method is very beneficial for thin, dry, flaccid, aging skins that cannot tolerate frictions in classical European/Swedish massage. The heating effect of the current will help to penetrate the nourishing ingredients of a rich massage cream. It is also beneficial post-operatively. We would usually wait four months following surgery and still want a doctors permission if in doubt. Due to the light effleurage movements, scar tissue can be worked over and this helps with healing. Delicate skin can be worked over lightly and the benefits of deeper massage still enjoyed. When the therapist has their hands in contact with the client’s skin, they are completing a circuit. The skin is then very gently massaged for 8-15 minutes. The client may experience a mild tingling sensation, the therapist may also feel this in the finger tips. One hand should always be kept in contact with the clients skin to ensure a circuit is complete. If both hands are removed during the treatment a “sparking” effect may occur. Optional to continue light massage movements over the skin for 5-10 minutes after current has been turned off. As indirect high frequency is performed while massaging the skin, products which facilitate slip and glide are recommended during this portion of the professional skin treatment. Gauze may be layered to create a spark gap Place electrode in working hand and make contact with finger before placing on skin surface Increase intensity and move lightly across the gauze in circular movements Superficial light movements have the greatest intensity Do not increase spark gap more than 1/4 inch (1/2 cm), continue working for up to five minutes Place finger on electrode, lift and remove from skin Switch machine off Following cleansing, analysis and exfoliation (extractions if necessary) we are ready to perform Direct High Frequency. High frequency can also be incorporated into the end of the treatment after removal of the masque. This is a guideline regarding the method of use. You will always need to check with your manufactures guidelines as to their specific recommendations. method of use IHF Prepare skin with a massage gel cream providing slip and glide Remove all jewelry from client and operator Client holds glass saturator electrode Operator places one hand on client’s forehead and turns up machine with other hand Both hands are now placed in contact and rhythmic massage commences over face and upper body One hand must remain in contact at all times to keep current flow continuous for approximately minutes Small frictions are beneficial around the mouth and eyes Release one hand from the client to turn down and switch off the machine This is a general overview of a professional treatment incorporating indirect high frequency. This is a guideline regarding the method of use, you will always need to check with your manufactures guidelines as to their specific recommendations. demonstration Undergraduate Instructor: Demonstration is designed to recap all information from today’s lesson. To save time, have your model complete the Dermalogica Consultation Card before the demonstration. Step-by-step Review the Dermalogica Consultation Card highlighting questions that may lead to possible contraindications to using electricity in the treatment. Cleanse eyes and lips with Soothing Eye Make-Up Remover. For stubborn eye make-up use PreCleanse on damp cotton. Rinse eyes and lips with damp sponges Cleanse the skin with PreCleanse , and perform Face Mapping making note of your extraction protocol Perform second cleanse under steam using client appropriate dermalogica cleanser. Remove all traces of cleanser with sponges. Galvanic Desincrustation – optional interactive demonstration Galvanic desincrustation is a great tool to help prepare the skin for extractions, using a negatively charged electrode applied to a desincrustation solution such as, Dermalogica’s Scaling Fluid, it will help soften sebum and impactions making extractions easier and more productive. Show students the galvanic machine – ask them which polarity the machine should be on when we perform desincrustation and call upon the students to list the effects this has on the skin Cover active electrode (-) (what therapist holds) in damp gauze or sponge that has been saturated with Scaling Fluid – optional interactive demonstration – guide student on preparation of the electrode Cover indifferent electrode (+) with damp sponge or gauze and place behind the client’s shoulder clipped to client’s wrist - optional interactive demonstration – guide student on preparation of the electrode Cover eyes with damp 2x2 gauze. Whip up Scaling Fluid to a foamy consistency and apply to clients face (T-zone only) - optional interactive demonstration – guide student on preparation of model Contact working electrode with clients face and turn machine on and slowly turn up - keeping electrode in contact with skin using a constant motion. Do not go higher than 3 milliamps. Do not stop moving the electrode during treatment and do not allow the skin to dry. Discuss process of desincrustation, and timeline of incorporating into the professional treatment – typically 3-7 minutes, but shortened for demonstration purposes Turn machine down and off before removing electrode from skin. Explain that extractions would follow this step in the treatment and then remove remaining desincrustation solution with damp sponges. Continued on following hidden slide
