1. Health Impact Council Description
Purpose: One of three distinct committees that convene community stakeholders in an ongoing process to identify priority needs, desired outcomes, and recommendations for funding within their assigned areas of focus.
Committee Size: Each Impact Council has (optimally) volunteers comprised of Board Members, issue experts, agency experts, neighborhood representatives, community partners and donors.
Time Commitment: 2 – 3 hours per month
Target Population: Adolescent children and their families.
Outcome: Increase the number of adolescent youth in the Healthy Fitness Zone by providing knowledge, skills, and counseling to address the root causes of obesity.
Modality: Provide one-on-one/group counseling to adolescent youth in acquiring the knowledge, skills, attitudes, to produce behavioral-change necessary for a healthier lifestyles (eating habits/physical activity).
Measurement:
Increased knowledge about healthy nutrition/physical activity skills, as evidenced by Pre- & Post- tests of program participants
Increase in the number of students who exhibit better eating habits as evidence by a food environment survey
Increase in the number of domains an individual students scores in the Healthy Fitness Zone
Increase in number of students with in the targeted school scoring in the Healthy Fitness Zone
