HOME HOSPITAL LOG WORKING TOGETHER FOR THE STUDENT

2 WHAT IS HOME HOSPITAL?? When a student has a diagnosed illness, surgery or injury that does not allow them to attend school regularly. When a student is receiving Educational services at home. The length of time that the student is on Home Hospital will vary per each individual case.

3 NAC Pupil receiving individual instruction at home or in medical facility. (NRS , ) 1. A pupil who has an illness or a disability which dictates that the requirement of compulsory attendance be fulfilled by individual instruction at home or in a medical facility under the supervision of a licensed teacher employed by the school district shall be deemed enrolled within that district if he or she is scheduled to receive at least 5 hours of instruction per week directly from the supervising teacher in the form of individual tutoring. The tutoring may be conducted by means of electronic telecommunication. 2. Each hour of teaching devoted to the individual instruction of the pupil is equivalent to a full day of attendance. More than 1 day of attendance may be accumulated during a single instructional session of more than I hour, but the number of days of attendance reported for the pupil for that school year may not exceed the number of school days in session during that school year. 3. A pupil with a disability receiving instruction pursuant to this section must receive the type and duration of instruction specified in the pupil's individualized education program, and all reporting of enrollment and attendance must be based upon the requirements of that program. 4. The pupil may be reported monthly as enrolled and in attendance at the school he or she would normally attend, or a separate monthly report may be submitted, marked "Individual Tutoring at Home or in a Medical Facility."(Added to NAC by Bd. of Education, eff ; A ; R033-99, )

4 Two types of Home Hospital Students
A student can be placed on a Home Hospital Plan through the 504 office. Or A student who has an active IEP plan will be placed on Home Hospital Plan through Sped.

5 Who works to get a Student on Home Hospital?
Student, Parent, Doctor School Counselor, School Nurse 504/SPED Home Hospital Team

6 Responsibilities for Counselors/504 Case Managers
Conduct a 504 team eligibility meeting. Complete HH form. Contact 504/HH department to request a HH tutor to be assigned. Introduce HH tutor to student’s teacher(s). Communicate to school secretary/registrar/attendance personnel the student will be on HH status. Communicate to school secretary/registrar/attendance personnel when the first meeting date for HH service will take place. Designate a place in front office for Home Hospital work to be picked up and dropped off. Homework will be picked-up and dropped off on Friday (unless permission is granted from the 504/HH dept. for a different day)

7 Responsibilities of Home Hospital Tutor/Teacher
Communicate to parents the role of a Home Hospital tutor. Arrange a first meeting date. Communicate the first meeting date to the counselor and the school office staff. Schedule the log sheet with Parents for two weeks at a time. Get each scheduled week signed. Provide a copy of the most recent log sheet to 504/HH office every Friday. Provide a copy of the most recent log sheet to the school secretary/registrar/attendance personnel during the pickup and drop-off of homework each Friday.

8 Roles for School Office Personnel
Adjust the Home Hospital Student’s attendance once the logs have been turned into the school. Notify counselor when the Home Hospital student is withdrawn/unenrolled due to attendance. Follow the Student Accounting process for maintaining log sheets on school SharePoint.

12 Who at a school site should be in charge of Home Hospital Logs?
It is up to each school site to determine who will process the Home Hospital Logs. Logs are scanned.. Logs are uploaded onto the School’s SharePoint

13 Steps to set up a SharePoint folder for Home Hospital Logs.
In SharePoint, create a folder called “Home Hospital”. Then create a subfolder called 504 (current school year) and a separate subfolder called Special Ed (current school year). Within the subfolder, create a student file label it with the student’s last name, first name. Logs should be labeled week 1, week 2, week 3 etc. and placed in the students folder.

16 Attendance A Student can only be marked AEL if he/she has been confirmed and served by the Home Hospital Program. Once the school has received the Home Hospital Logs the student’s Attendance can be changed to AEL. The AEL can be assigned to the current week on the log and the following week if a plan has been noted on the log and it has been signed by the parent. AEL code can only be pre marked one week in advance. AEL should also be used if a student has temporarily been assigned to Jan Evans.
