Your Community Health Center serves >5,000 patients in Phelps, Dent, Maries, Crawford, and Pulaski counties. Family Practice and Dental Clinic and School- based Health Care Clinic. The mission of Your Community Health Center is to provide access to high quality, compassionate, patient-centered healthcare to everyone regardless of financial, cultural or social barriers. No routed public transportation system offered- no city bus routes Logisticare for Medicaid patients We team up with local transportation companies such as SMTS (Southeast Missouri Transportation Service), Licking Bridge Builders, and OATS Transit (Operating Above the Standard Transit) Local taxi services are utilized Transportation Outlets in Phelps and Surrounding Counties for Patients
