What is the VI-SPDAT Vulnerability Index-Service Prioritization Decision Assistance Tool Developed by OrgCode and Community Solutions Triage Tool

18 Why Conduct a VI-SPDAT? Assist in identifying the most appropriate resources for those experiencing a housing crisis Prioritizes Housing for those who are most vulnerable Assist in identifying the level of support services that may be needed post-housing. Helps determine a person’s acuity level and their level of vulnerability in regards to their homelessness situation.

You will only need to re-complete a VI-SPDAT IF: A person was exited from Coordinated Entry in HMIS due to being housing or inactive AND are re-entering the system because they are LITERALLY HOMELESS and want to work towards housing.

The Metro Homeless Impact Division in partnership with MTA was able to secure 500 Annual Bus Passes for adult persons who are experiencing literal homelessness and are identified as chronically homeless or at risk of becoming chronically homeless.

47 IPV/DV Coordinated Entry & RRH
Mary Parrish Center was awarded grants for clients who have or are experiencing Interpersonal/Domestic Violence. One grant is to implement a parallel CE process for IPV/DV survivors and the other two are for Rapidly Rehousing those clients with these experiences.

48 What is Interpersonal Violence?
Interpersonal Violence Definition (IPV): is the intentional use of physical force or power, threatened or actual, against a person or group that results in or has a high likelihood of resulting in injury, death, psychological harm, maldevelopment, or deprivation. IPV can be committed by a person who is or has been in a romantic relationship, spouse or partner, family member, cohabitant, or household member including a roommate. Borrowed from Appalachian State University.

49 DV CE vs. General CE More considerations will be made regarding confidentiality and safety regarding the interpersonal violence they have or are currently experiencing Clients who go through DV CE WILL NOT be entered into General CE via HMIS.

52 DV Assessment A DV CE Assessment Tool that emphasizes the unique risks faces by survivors has been created through a partnership with Mary Parrish Center, YWCA, Morning Star, The Family Safety Center, and Metro Homeless Impact Division.

56 Youth and Young Adult Diversion
Through the Youth Homelessness Demonstration Program (YHDP), Oasis was awarded with a grant to provide YYA 24 years of age and younger with Diversion Assistance.

57 YYA Diversion YYA Diversion will work to find other permanent connections and potential resources for YYA who are imminently at risk or have already become homeless. Once placement is found, case management will be provided for up to 6 months to ensure stability. Rental Assistance is NOT included in this resources

62 The Next Door Freedom Recovery Community
The Freedom Recovery Community is PERMANENT apartment living for women and their children. This community provides the opportunity to continue recovery services onsite within a supportive peer environment.

Woman has to be 18+ years of age Addiction to a substance, but demonstrates at least 30 days sober Has a MH Diagnosis and history of treatment Single or have up to 3 minor children. OR Receiving SSI or SSDI; but willing to work at least Part-Time Employed working at least 30 hours a week or willing to work 30 hours per week Agreeable to participate in required group meetings and CM sessions Pass a blood alcohol test and urinary drug screen upon admission

Once the referral is made the agency will continue the communication

69 Critical Time Intervention (CTI) Case Management
The Metro Homeless Impact Division and Centerstone have a contract to provide Critical Time Intervention (CTI) case management where anyone who has been housed and has gone through the CE process can receive 9 months of case management for housing stabilization The program serves about 60 people at any given time. **This contract is limited and focuses on prioritizing people with high needs who are not eligible to receive support services/case management at partner organizations.

CTI is a time-limited, focused case management approach designed to support people as they transition into housing from homelessness CTI generally lasts for 9 months

CLIENT MUST CONSENT AND WANT THE SERVICES Upload Referral Form in HMIS (within 30 days of being housed) Deon Trotter Deon will send referral to CTI team to be assigned Once assigned Deon will let you know the name of the new CM The CTI CM will schedule a warm handoff with you and the clt.

73 Once a Client is Housed Please make sure to complete the CE Exit via HMIS This is imperative for our community to know how many individuals/families are exiting homelessness into permanent housing.
