SUMMARY OF 2017 IMPROVEMENTS Medical Plan Continue to provide high quality comprehensive medical plans with low premiums and low out-of-pocket costs Only small increases to employee premiums (at most, $25/month) Increased the major medical coinsurance in PPO from 80% to 90%, which will lower out-of-pocket costs to employees for major services like hospitalization Example Condition Overall Cost Deductible Co-insurance Patient Responsibility Arthroscopic Knee Surgery $17,000 $400 Old: 80% $3,320 New: 90% $1,660 Total Savings from Increased Co-Insurance Dental Plan Orthodontia lifetime max increasing by $500 from $1,500 to $2,000 Major Expenses coinsurance increasing from 50% to 60%. For example, for a crown with billed charges of $1,000, your out-of-pocket costs will be $100 lower. No increase to employee premiums

New for 2017 Match Formula 50% of first 6% 75% of first 6% Maximum Match 3% 4.5% Match enhancement will better help you prepare for retirement Rule of thumb: Fidelity recommends saving 13-15% of your pay in order to have enough money for retirement With this change, contributing 6% gets you to 10.5% total contribution to retirement New match takes effect on earnings and contributions from Jan. 1, 2017 (i.e. the Q1 match which is funded after the end of Q1) Existing graded vesting schedule (three year graded) remains Enhanced Distribution Options for Retirees Today, there are limited options for retirees to receive cash – you may either cash out the entire lump sum or rollover to IRA We are enhancing options to provide: Partial or periodic cash withdrawals – amount and frequency is determined by participant

Review your existing elections: Increase Opt Life by up to $40,000 in $10,000 increments No restriction on increasing AD&D Elections can not exceed five times your annual salary Don’t forget about the premium reductions of up to 10% on Voluntary Life and Long Term Disability

ACCIDENT INSURANCE MetLife provides lump sum payments to the employee in the event of a variety of different accidents that result in emergency room treatment or hospitalization Just a few types of accidents that are covered under the plan: Fractures Burns Concussions Ambulance (air or ground) usage benefits Hospital stay benefits Coverage is meant to compliment the medical plan and help offset out-of- pocket costs in the medical plan Two Plan Options are Available – High Plan and Low Plan

CRITICAL ILLNESS INSURANCE
MetLife provides lump sum payments if you suffers from one of many serious or critical illnesses Coverage for 22 conditions. A few of the serious or critical illnesses covered: Heart Attack Stroke Cancer Diagnosis Organ Transplant End Stage Renal Failure Two Plan Options are Available – High Plan and Low Plan

VIRTUAL VISITS: TELEMEDICINE
You can see and speak to a health care provider 24/7 using a mobile device or computer, all from the convenience of your home or office First visit must be by video chat (from phone, tablet, pc) Most visits last only ten minutes Prescriptions are sent to your choice of local pharmacy These Virtual Visits can help with most common conditions: Allergies Cold/Cough Fever Nausea Sinus Infection Billed charges range from $40 - $60 vs. $125 - $200 for primary care/urgent care visit PPO Plan members: $20 PCP copay will apply Account Based Plan members: billed charges subject to deductible/coinsurance
