Of 119 patients that presented with supratentorial tumors and tumor associated seizures, 41 underwent intraoperative ECoG-guided surgery. Of these, 26 patients’ intraoperative ECoG readings were negative after lesionectomy; 15 continued to display abnormal spike activity, 12 of which underwent additional resection. In the remaining 3 patients, ECoG signals arising from eloquent cortex were deliberately spared. Outcomes were compared between patients undergoing lesionectomy alone vs lesionectomy plus electrocorticography-guided surgery (diagonally striped bars). A, When comparing Engel Class I and II to Classes III, and IV at 6 mo, 1, 2, and 5 yr postsurgery, no differences were seen in seizure freedom or control. B, Examining the use of AEDs per patient, up to 5 yr after surgery, there was no significant association between ECoG and the number of postoperative AEDs. Abbreviations: AEDs, antiepileptic drugs; ECoG, electrocorticography.
