2 Azara Immunization Registry
I like to use the registry as an outreach tool. For many of the measures, you can use the clinical performance measure detail report and sort by whether the patient met the numerator. That doesn’t work for immunizations…they are already past the age point for vaccinating when they are included in the denominator. For example, they would need to have already turned 2 years old to be included in the measure. Default is for one week, I prefer to look at trailing 12 months

3 I like to eliminate most of the demographic info on my worksheet, so mine may look differently than you are used to seeing on your report. Sort out the patients who have completed their immunizations.

4 The registry report includes all ages of children, so if you are focusing on a particular set of immunizations, sort by age range. For example, if I want to look at the 2yo immunizations, sort for children less than 2 years old:

5 Next I will sort A to Z for “Months Until 2”
Next I will sort A to Z for “Months Until 2”. This lets me prioritize those patients with a tight timeline to complete their series.

6 It’s nice to be able to see which vaccines are missing

7 In some cases, we will see that we cannot make the cutoff to meet the measure. The child on the first line will be turning 2 in 1 month….there’s not enough time to get the series completed before the child turns two. We will still try to get this child in, but already know we won’t meet the measure for this one. The second highlighted child will turn 2 in 8 months, so possibly, with a catch up schedule, this child may be able to get up to date in time to meet the measure.

8 It is important to review the list before calling patients
It is important to review the list before calling patients. In this first patient (in yellow) it looks like the patient hasn’t had any vaccinations documented. We need to investigate the chart and CHIRP. Sometimes, the child has a record that was scanned into the EMR, but the support person didn’t get the immunizations charted in the structured data fields. If this is the case, they will need to do that work at this point. EClinicalWorks does not have a bi-directional feed yet so we also have the support person check the CHIRP record to make sure those records match what we have in our EMR. If anything is missing, they are asked to enter that documentation.

9 I like to check the columns for next scheduled appointment.
If the child is already scheduled in a reasonable time frame in which to do the immunizations, we just add a note to the appointment message or to the chart to remind provider team to vaccinate at the visit. If the child is not already scheduled, we will call the parent to let them know their child is due for vaccines and try to get them scheduled as timely as possible.
